About this file

This document is a Contract Amendment for the Arizona Department of Economic Security (DES), involving LanceSoft Inc. as the contractor. The amendment modifies Contract Number CTR076897, which originally covered Nursing and Habilitation Services for Individuals with Intellectual Disabilities at Intermediate Care Facilities and State-Owned Group Homes. The amendment now expands the contract's scope to include the Arizona Department of Veterans' Services (ADVS), specifically adding nursing service provisions for four Veterans' Homes located in Maricopa and Pima Counties.

The amendment introduces new requirements for the contractor, including providing healthcare and supporting active treatment services to veterans' home residents. It establishes a framework for service delivery through mutually approved Task Orders that will outline service requirements, deliverables, timelines, and payment terms. Additionally, the amendment adds comprehensive incident reporting requirements, mandating that the contractor submit incident reports according to specific medical policy manuals and immediately report sentinel events via the AHCCCS QM Portal within one business day of occurrence. The amendment maintains all other existing contract terms and conditions, with final execution to be completed through electronic approval in an application system.
