The contract is effective January 1, 2026, with an initial three-year term and the option for two one-year renewals, not to exceed a total of five years. The DHMO plan will cover approximately 9,052 subscribers across three enrollment tiers: subscriber only, subscriber plus one dependent, and subscriber plus two or more dependents. Monthly premium rates range from $10.92 to $27.68 per tier, with projected annual premiums increasing from $1,509,778 in 2026 to $1,791,562 in 2030. The plan includes comprehensive dental services with varying copays, performance guarantees, and specific requirements for network access, claims processing, customer service, and administrative support, with Cigna required to maintain on-site representatives and meet detailed service standards.
