About this file

This document is a Contract Amendment issued by the Arizona Department of Economic Security, modifying contract CTR076171 with Family First Assisted Living Home LLC. The amendment specifically addresses changes to the incident reporting requirements for an assisted living facility contract, involving the Division of Developmental Disabilities (DDD) and the Arizona Health Care Cost Containment System (AHCCCS).

The amendment updates Section 7.3 of the original contract, detailing new incident reporting protocols. Contractors are now required to submit incident reports as defined in various policy manuals, with specific emphasis on reporting sentinel events. Sentinel events must be reported immediately via phone (602-375-1403 or 1-855-375-1403) and then submitted through the AHCCCS QM Portal within one business day. The amendment provides multiple user guides and links for accessing the QM Portal, ensuring clear communication channels for incident reporting. All other terms and conditions of the original contract remain unchanged, and the amendment will be executed through electronic approval.
