About this file
This document is a Contract Amendment Summary issued by the Arizona Department of Transportation (ADOT) Procurement division, involving Pavement Sealants & Supply Inc. as the contractor. The amendment is for Contract Amendment Number One (1) related to Asphalt Cold Patch Materials and was revised in April 2020.
The amendment modifies two key sections of the original contract: Specifications and Special Terms and Conditions. Specifically, it mandates that each delivered load must have a certified weigh ticket indicating gross, tare, and net weight, and ensures delivery terms are F.O.B. Destination with freight collect. A new line item (CTR070195-2) for Freight was added to the price list, and the contractor is now required to include estimated freight costs in quotes, which will be reimbursed by ADOT as a pass-through cost without markup. All other contract terms remain unchanged, maintaining the original agreement's core provisions.
