About this file

This document is a Contract Amendment issued by the Arizona Department of Economic Security (DES) for Contract Number CTR066099, Amendment Number Six. The amendment involves Las Palmas Assisted Living Home, a contractor providing assisted living facility services. The document specifically addresses modifications to the incident reporting requirements section of the existing contract.

The amendment updates the contractor's obligations for incident reporting, requiring submission of incident reports as defined in multiple policy manuals, including the DDD Medical Policy Manual and AHCCCS Medical Policy Manual. Specifically, the contractor must report sentinel events immediately by phone to the Division (with 24/7 available phone lines at 602-375-1403 or 1-855-375-1403) and then submit detailed reports via the AHCCCS QM Portal within one business day of an event's occurrence. The amendment provides multiple reference links for portal access, user guides, and additional procedural information. All other contract terms and conditions remain unchanged, and the amendment will be executed through electronic approval.
