About this file
This document is a Contract Amendment issued by the Arizona Department of Health Services for Contract No. CTR047091, involving Concentric Healthcare Solutions, LLC as the contractor. The amendment is specifically for Registry Staffing Services for the Arizona State Hospital and is Amendment No. 3 to the original contract.
The amendment introduces two key incentive pay provisions for healthcare staff during the COVID-19 pandemic: a 15% crisis pay incentive for time worked at the State Hospital, and a 25% hazard pay incentive for staff assigned to a COVID-19 Unit. The contractor is permitted to increase its hourly billable rate to the hospital to account for these incentive rates, with the hospital agreeing to notate timecards to indicate when staff work on a COVID-19 unit. The effective date of these changes is June 19th, 2020, and the price sheet has been updated to reflect two new line items (38.0 and 39.0) for these incentive percentages, both with a unit price of $0.00.
