About this file
This document is an Amendment (Amendment No. 5) to a mandatory use contract issued by the State of Ohio's Department of Administrative Services (DAS), General Services Division, Office of Procurement Services. The contract involves Segue Consulting of Denver, Colorado, and is specifically for the Ohio Department of Health, with the contract number CSP902021 and index number DOH008.
The contract is for Healthcare Reform and Stakeholder System Change Consulting services, originally effective from 08/27/20 to 06/30/21. This fifth amendment extends the contract for an additional 12-month period from 07/01/25 through 06/30/26, maintaining all previous pricing, terms, and conditions unchanged. The contract appears to be a recurring annual renewal, as evidenced by the summary of previous amendments showing consistent 12-month extensions since the original contract's inception. The contract is available on the DAS web site at www.ohio.gov/procure and is a mandatory use contract for the Ohio Department of Health, with no guaranteed purchase volume specified.
