About this file
This document is Amendment No. 6 to a Mandatory Use Contract issued by the Ohio Department of Administrative Services (DAS), General Services Division, Office of Procurement Services. The contract involves Strategic Research Group, Inc. of Columbus, Ohio, and is specifically for the Ohio Department of Health. The amendment is part of a contract originally issued under Request for Proposal (RFP) No. CSP901921, which was first awarded on 06/22/20.
The contract covers Evaluation Services for Tobacco Use Prevention and Cessation Programs, with the current amendment extending the contract for one additional month from 07/01/25 through 07/31/25. This is a continuation of previous renewals, including a 24-month renewal from 07/01/23 through 06/30/25 per Amendment 5. The contract is a Requirements Contract, meaning the Ohio Department of Health can purchase contracted services in any amount and at any time, though the state makes no guarantee of a specific purchase volume. All previous pricing, terms, and conditions remain unchanged with this amendment. The full contract and its amendments are available on the DAS website at http://www.ohio.gov/procure.
