About this file
This document is Amendment No. 6 to a Mandatory Use Contract (Contract No. CSP900221) issued by the State of Ohio's Department of Administrative Services (DAS), specifically for the Human Resources Division (HRD) and Benefits Administration Services (BAS). The contract is with Personify Health Inc., located in Providence, Rhode Island, and involves an Employee Population Health Management (PHM) Program. The amendment is effective from 07/01/25 through 06/30/26 and includes three subcontractors: Quest Diagnostics, SilverCloud Health Inc., and WellBeats, Inc.
The amendment renews the existing contract for an additional 12 months and adds the Next Steps Consult Program, a concierge-style engagement conversation to help employees understand their health status by reviewing Health Risk Assessment (HRA) and biometric screening results. The contract includes a new prohibition on offshore services, preventing state data from being sent, accessed, or stored outside the United States without a specific waiver. The payment terms are Net 30, and the contract is available on the DAS website at www.ohio.gov/procure. This is the sixth amendment to the original contract, which was initially awarded following a Request for Proposal (RFP) that opened on 10/09/19.
