About this file
This document is Amendment 1 to Contract CSP025543 between the State of Ohio, Department of Administrative Services, and RVO Health, LLC, relating to the Ohio Tobacco Quit Line services. The amendment was executed on 02/03/25 and modifies the original contract, which was initially set to be effective on 03/01/2025.
The amendment updates the contract to include new pricing for nicotine replacement therapy products, specifically adding Nicotine Patch courses of 4, 8, and 12 weeks at $52, $100, and $145 respectively, and NRT Gum and Lozenge boxes at $30 and $20. The contract now requires the contractor to provide three months of runout services after contract termination or expiration, continuing to serve Ohio Tobacco Quit Line participants who registered prior to the termination date. The amendment removes previous line items for 2-week courses of Nicotine Gum and Nicotine Lozenges. All other terms, conditions, and pricing remain unchanged, with the amendment effective for all orders placed on or after 02/03/25.
