MUTUALLY AGREED UPON CLARIFICATIONS / MODIFICATIONS TO THE RFP
MetroHealth proposes to expand the subcontracted services with the Legal Aid Society of Cleveland (LASC)from 65% to 150% (60 hours/wk) while increasing both the Project Manager's and OB Liaison's effort from 10% each to 15% each (6 hours/wk). We anticipate that LASC will provide a minimum of 1200 15-minute periods over the course of the 1-year grant.
This expansion will also permit expanded geographic scope from the current MetroHealth MLP sites providing OB and Pediatric services (MetroHealth's Broadway, Buckeye, Main Campus, Ohio City, and Old Brooklyn Health Centers), to include OB and Pediatrics support care practices at health centers in these communities: Cleveland Heights, Glenville, Brooklyn, Parma and Bedford. Most of these communities align with the high-risk zip codes identified by First Year Cleveland as having the highest infant mortality rates.
