About this file

This document is Amendment No. 4 to a Mandatory Use Contract issued by the State of Ohio's Department of Administrative Services (DAS), General Services Division, Office of Procurement Services. The contract involves the Program of All-Inclusive Care for the Elderly (PACE) Expansion and is specifically with Acutecare Health Systems LLC, located in Oceanport, New Jersey. The contract was originally awarded based on Request for Proposal (RFP) No. SRC7632 and is available to the Ohio Department of Aging.

The contract (CSP018727) was originally effective from 12/15/23 to 06/30/25 and has now been renewed through 06/30/26 via this amendment. The amendment maintains all existing prices, terms, and conditions while extending the contract period for an additional twelve months. Previous amendments to the contract have included updates to the staffing plan, financial questionnaire, and contract fund spending dates. The contract appears to be a requirements contract, with the state making no guarantees about the volume of services that will be purchased, but providing Acutecare Health Systems LLC the opportunity to provide PACE expansion services to the Ohio Department of Aging.
