About this file
This document is Amendment No. 3 to a Mandatory Use Contract for Pharmaceutical Prime Wholesaler Services between the State of Ohio Department of Administrative Services (DAS) and Amerisourcebergen Drug Corporation. The contract is part of a consortium arrangement with MMCAP Infuse serving as the lead entity, and it involves the Ohio Department of Mental Health and Addiction Services and the Ohio Department of Veterans Services. The contract was originally awarded through Bid Number 27147 and is administered by MMCAP Infuse.
The contract provides pharmaceutical prime wholesaler services with an original effective period from 11/01/22 to 10/31/23, and has been subsequently renewed. This amendment extends the contract for an additional 3 months, effective 05/01/25 through 07/31/25, and updates the contractor's contact information. The contract allows Ohio state agencies to purchase pharmaceutical supplies and services as needed, with no guaranteed volume commitment. The contract follows the terms and pricing of the MMCAP Infuse Master Price Agreement, and the remittance address for Amerisourcebergen is located in Chicago, Illinois, with the contractor's contact being Jennifer Bennett.
