About this file
This document provides instructions for the Commonwealth of Massachusetts Executive Office of Health and Human Services Massachusetts Rehabilitation Commission (MRC) regarding the CORI (Criminal Offender Record Information) verification process for individual providers. The instructions outline three options for completing the CORI Acknowledgement form verification: in-person at an MRC office, through a remote verification call with MRC staff, or by having the form notarized. Providers are instructed to submit the completed and verified CORI form to the MRC, and to indicate the specific RFR (Request for Response) they have applied for.
The related state and local contract opportunity is for the MRC's Community Living and Vocational Rehabilitation Divisions to recruit qualified Masters and Doctoral level clinicians to provide assessment, consultation, and treatment services to individuals with disabilities in the state of Massachusetts. No additional details are provided regarding the project scope, quantities, timelines, or contract terms.

Please complete and return the Cori Acknowledgement form attached to this procurement. Before this form can be submitted to the Cori Board, it must be verified. The verification can be completed at a Massachusetts Rehabilitation Commission (MRC) office, or may be verified during a remote call with a member of the MRC staff, or in the presence of a notary public.
For onsite verification by MRC staff Contact your closest or most convenient MRC office and set up a time to go in. Please bring a government-issued photo ID (driver’s license, passport, etc.) A member of the MRC staff will meet with you, view your photo ID and sign the bottom of the CORI form. They will also make a copy of your ID and attach it to the form.
Please email completed CORI form to: Karyn.Weizel@mass.gov
For remote verification by MRC staff Contact an MRC area office to arrange a time for a remote call, with video. The MRC staff member will complete the Remote Verification Form attached to the CORI form
For verification by a notary public Contact a notary public for an appointment. Forward the notarized form to MRC at the address above.
As a reminder, when submitting your verified CORI form, please indicate the RFR that you have applied for.
Thank you for your cooperation.
