About this file

The document is a CORI Acknowledgement Form from the Massachusetts Rehabilitation Commission, which is registered to receive Criminal Offender Record Information (CORI) to screen current and prospective employees, contract providers, volunteers, and interns. The form outlines the authorization process, noting that the CORI check is valid for one year, and that the Commission may conduct subsequent checks with written notice. Applicants must provide personal information, including name, date of birth, social security number, and address history, which will be verified against government-issued identification.

The related state and local contract opportunity is for independent (individual) care providers selected by MRC consumers, rather than for organizations, in the jurisdictions of Suffolk County and the City of Boston, Massachusetts. No additional details are provided regarding the products or services, quantities, response dates, award information, or contract term.
