About this file
This document is a Contract Amendment issued by the Arizona Department of Child Safety's Office of Procurement, specifically Amendment #21 to Contract #CTR062904. The amendment pertains to a statewide contract for facility-based services, involving multiple residential facilities located across various cities in Arizona, including Phoenix, Tucson, Mesa, and other metropolitan areas.
The amendment adds two new facilities, Tuckey 15 and Tuckey 16, both located in Phoenix, Arizona, each with a capacity of two (2) beds. With these additions, the total contracted capacity increases to sixty-seven (67) beds across 27 different facilities. The contract covers a network of residential care locations with varying bed capacities, ranging from one to five beds per facility. The effective date of this amendment is October 22, 2024, and all other terms and conditions of the original contract remain unchanged. The facilities are geographically distributed across Arizona, suggesting a statewide approach to providing residential care services, likely for child welfare or similar social services.
