About this file

This document is a Contract Amendment (Amendment #4) for Contract #CTR071405 issued by the State of Arizona's Department of Child Safety, located in Phoenix, Arizona. The amendment involves modifications to the existing contract's Special Terms and Conditions and Scope of Work, addressing background checks, drug testing requirements, and other contractual provisions.

The amendment includes two primary updates: first, a revision from an October 2024 amendment that updated Special Terms and Conditions with new requirements around background checks, including provisions that the Department will not pay for services if contractors are not compliant with background check requirements. The second update, effective April 1, 2025, modifies Scope of Work sections related to drug testing and background checks, specifically requiring contractors to have clear pre-employment drug tests, quarterly random drug testing, and ensuring all paid and unpaid adults and subcontractors complete Level 1 fingerprint cards and background checks before providing services to children. The amendment becomes effective upon execution in the Arizona Procurement Portal or by August 1, 2025, whichever occurs earlier.
