About this file

This document is a Contract Amendment (Amendment #4) for Contract #CTR071409 issued by the State of Arizona's Department of Child Safety (ADCS), located in Phoenix, Arizona. The amendment involves modifications to the existing contract's Special Terms and Conditions and Scope of Work, with revisions processed through the state's e-procurement system.

The amendment focuses on several key modifications, including updates to background check requirements, drug testing protocols, and personnel screening procedures for paid and unpaid adults and subcontractors working with children. Specifically, the contract now mandates that all personnel must have a Level 1 fingerprint card, Central Registry Clearance, and complete background checks prior to hire. Additional requirements include having a clear pre-employment drug test, submitting a signed Background Check Authorization form, and undergoing random quarterly drug testing aligned with ADCS Drug Testing Policy. The initial contract prices will be honored for a one-year term, and the amendment becomes effective upon execution in the Arizona Procurement Portal or by August 1, 2025, whichever occurs earlier.
