Statement of Need:
The U.S. Food and Drug Administration (FDA), National Center for Toxicological Research (NCTR), profiling proteins in mouse heart tissue and plasma samples treated with sunitinib using Data-Independent Acquisition (DIA), a label-free quantitative next-generation proteomics technique to aid in discovery of novel proteins expressed in heart prior to the onset of cardiac injury. These proteins can be used as early predictive biomarkers of cardiotoxicity and may prove valuable in identifying cancer patients or survivors susceptible at risk for future heart damage and/or designing new interventions to minimize cardiotoxicity tailored to susceptible sub-population. Therefore, quantitative proteomic profiling with high precision, accuracy, and reproducibility is required to accomplish our goal.
Scope:
FDA/NCTR/Division of Systems Biology requires identification of early predictive biomarkers of sunitinib-induced cardiac damage. Sunitinib is a tyrosine kinase inhibitor approved for treatment of cancers such as renal cell carcinoma, imatinib-resistant gastrointestinal stromal tumor, and pancreatic neuroendocrine tumors. Recent clinical studies have shown that patients treated with sunitinib are at increased risk of developing cardiac dysfunction. The most common adverse effects of sunitinib are decrease of left ventricular ejection fraction (LVEF), hypertension and QT interval prolongation in patients. However, the multi-target nature of sunitinib at therapeutically relevant concentrations makes it difficult to identify the precise mechanism of cardiotoxicity. Many aspects of sunitinib-induced cardiotoxicity such as on/off target toxicities and the interactions of related pathways are still not fully understood. Also, there is also a lack of information related to sex-based differential cardiotoxicity induced by sunitinib.
Currently, there are no clinical biomarkers to predict sunitinib-induced cardiotoxicity during cancer therapy. Thus, there is a need for identification of novel biomarkers capable of predicting early cardiotoxicity induced by sunitinib. Use of early cardiotoxicity biomarkers will prove valuable in identification of cancer population at risk for future heart failure so that effective cardioprotective interventions can be implemented sooner to minimize or prevent sunitinib-mailto:howard.nesmith@fda.hhs.gov induced cardiotoxicity. This will enhance FDA’s goal to protect and promote individual health.
These early biomarkers will allow accurate prediction and minimize the risks associated with FDA-regulated products.
Additionally, quantitative profiling of proteins using highly sensitive high throughput sequencing technology in heart tissue and plasma samples from mice treated with sunitinib will aid in discovery of novel proteins expressed in heart prior to the onset of myocardial injury or pathology. These proteins can serve as early predictive biomarkers of cardiotoxicity and may have potential in identifying cancer patients or survivors at risk of future heart damage in clinics.
Results from the quantitative proteomic profiling in heart tissue and plasma will facilitate identification of novel early preclinical markers of sunitinib-induced cardiotoxicity.
Minimum Requirements:
• Services shall must use the most advanced next generation Mass Spectrometry (MS)-based quantitative proteomic profiling technology such as label-free Data-Independent Acquisition
• MS-based quantitative proteomics is required since it includes a growing set of ancillary technologies, that provide a means for high-throughput characterization and quantification of proteins in a biological sample
• Mass Spectrometry (MS)-based quantitative proteomic profiling technology such as label-free DIA is required as it allows increased identification of protein numbers with higher accuracy and reproducibility
• Spectra library construction shall be done following the protocol and must quantitate all the proteins (2,000~10,000) within as many samples in a single assay. Label-free DIA technology shall include protein extraction, enzymolysis, library preparation and analysis on the High-Performance Liquid Chromatography (HPLC) coupled with high resolution high mass acracy Orbitrap Eclipse™ Tribrid™ Mass Spectrometer
• No more than 5mg of tissue power or 100µl of plasma will be used for these assays
• Downstream data analysis shall be performed including QC and quantitation
• The data delivery will include FASTQ format file with protein annotations which will be copied into an FDA approved hard drive and delivered FDA through commercial carrier
• All samples shall be profiled under the same conditions at the same time to obtain reliable results
• All data shall be provided to NCTR in FASTQ format with protein annotations within 12 weeks after receiving the samples
• All remainder of the heart tissue and plasma samples if any shall be shipped back to
NCTR in dry ice using commercial carrier paid by the service provider
Quantitative Proteomic Profiling Requirements:
• Shall utilize Data-Independent Acquisition (DIA), a label-free quantitative next-generation proteomics technique which provides highly consistent quantitation and broad proteome coverage compared to stable isotope labeling method
• Shall be able to handle large samples and quantify all the proteins (2,000~10,000) within sample sets, which require accurate and reproducible quantitation
• Shall provide reliable, reproducible, and quantitative analysis of 2,000~10,000 proteins in a single assay
• Shall be performed on the most cutting-edge instrumentations that provide more deep quantification on low abundant proteins and provide better quantitation accuracy
• Shall be able to quantify proteins in less amount of sample (<5mg for tissue samples and <100μl for plasma samples) with high precision
• Shall be able to quantify proteins in heart tissue as well as plasma sample for comparability
