About this file

This document is Amendment #3 to Agency Term Contract ATC-21-008 between the Florida Department of Corrections (the Department) and Hawkins, Inc. (the Contractor), governing the procurement of wastewater and water treatment chemicals. The amendment involves modifications to the existing contract, including updates to contract management personnel, public records handling, and scrutinized companies certification.

The contract covers the purchase of various chemical treatment products across four regions, including Sodium Permanganate, Phosphate, Aqua Hawk, Ammonium Sulfate, Sodium Hypochlorite, Hydrated Lime, and Orthophosphate/Polyphosphate products. Pricing is firm and includes handling, shipping, and environmental fees, with unit prices ranging from $2.35 to $787.25 depending on the product and unit of measurement. The estimated annual quantities vary by region, with Sodium Hypochlorite 12.5% showing the most significant volume (ranging from 100 to 45,510 gallons). The total estimated annual value for each region ranges from approximately $25,529.50 to $170,755.75. The contract includes provisions for potential unilateral cancellation and requires the contractor to comply with public records laws and anti-boycott certifications.
