About this file

This document is Amendment No. 2 to AHCA Contract No. MED210, an agreement between the State of Florida's Agency for Health Care Administration (AHCA) and Myers and Stauffer LC. The amendment modifies an existing contract between these two parties, with Myers and Stauffer serving as the vendor for AHCA.

The amendment introduces an Ad-hoc Project for Year 1 Implementation Support, which includes five specific tasks: developing a distribution plan for funding, creating and testing funding applications, establishing policies and procedures for application submittal, incorporating application data into a tracking database, and reviewing applications for eligibility. The total cost for these ad-hoc tasks is $535,668.00, increasing the total contract amount to $3,685,668.00. The tasks have specific due dates ranging from November 30, 2021, to September 30, 2022, with individual unit costs for each task. The project aims to support low-income recipients and help divert them from hospital placement by creating a comprehensive funding and application process.
