About this file
This is a Scope of Services contract between the Agency for Health Care Administration (AHCA) in Florida and Humana Medical Plan for the operation of Medicare Advantage Dual Eligible Special Needs Plans (DSNPs) throughout Florida. The contract establishes the terms under which Humana will provide coordinated Medicare and Medicaid benefits to dual-eligible beneficiaries across nine authorized service regions (A through I) spanning the entire state. The contract is effective January 1, 2026, through December 31, 2028, and includes multiple plan benefit packages (PBPs) designated as either Highly Integrated Dual Eligible (HIDE) SNPs or Fully Integrated Dual Eligible (FIDE) SNPs, with specific service areas defined by county.
The contract requires Humana to provide comprehensive Medicaid covered services including behavioral health services, long-term care and supports, home health services, medical supplies and equipment, prescription drugs, emergency services, hospital and nursing facility care, and various specialty services referenced in the Florida Medicaid Coverage Policies and fee schedules. Humana must coordinate all Medicare Part C and D benefits with Medicaid services under a single managed care organization, implement integrated care management and case coordination for high-risk beneficiaries, and ensure cost-sharing protections for qualified Medicare beneficiary categories. The FIDE SNP (Plan Benefit Package H1036-280) specifically covers 365 days of all-inclusive nursing facility services annually and provides capitated Medicaid payments under the companion FP129 Long-Term Care program contract. Pricing for the 2026 contract year is identified as "To Be Determined" pending CMS approval, and rates are established separately for different Medicaid eligibility categories (QMB, QMB Plus, SLMB Plus, and FBDE) across the nine service regions.
