About this file
This document is Amendment No. 1 to AHCA Contract No. FP083, an agreement between the State of Florida's Agency for Health Care Administration (AHCA) and CarePlus Health Plans, Inc. The amendment modifies an existing contract for a Medicare Advantage Dual Eligible Special Needs Plan (D-SNP) in Florida, covering Medicaid services for dual-eligible beneficiaries across eleven regions of the state.
The amendment updates several key contract elements, including revised performance standards and liquidated damages, changes to service coverage, and updated capitation rates for different Medicaid eligibility categories. The capitation rates vary by region and eligibility type, ranging from $7.40 to $94.31 per member, with different rates for Qualified Medicare Beneficiary (QMB), QMB Plus, and other dual-eligible categories. The contract covers Medicaid services not covered by Medicare, including various medical services such as behavioral health, dental, durable medical equipment, and nursing facility services, with specific provisions for enrollees with AIDS and other special healthcare needs.
