within budget. Volkert’s field office for this project will be located at 1255 Office Park Drive in Chipley, Florida.
AWARENESS AND UNDERSTANDING OF PROJECT SCOPE
1) UNDERSTANDING OF INTENT AND SCOPE OF
CONSTRUCTION PROJECT
Group 26-F contains multiple projects in Panama City, including SR 75 (US 231) from SR 30 (US 98) 15th Street to CR 2312 Baldwin Road and SR 368 (23rd Street) from SR 390 to SR 75 (US 231).
The two projects are similar in scope and are designed to improve the rideability of the existing roadway and pedestrian access.
SR 75 (US 231): This project consists of FM No. 450810-1-62- 01 and is located along SR 75 (US 231) from SR 30 (US 98) 15th Street to CR 2312 Baldwin Road for 2.928 miles.
The project scope includes:
• Milling and resurfacing of the travel lanes, median lanes, and side streets;
• Pedestrian/ADA improvements;
• Sidewalk construction;
• Signalization;
• Lighting;
• Mast arm/drill shaft construction; and
• Signage and pavement markings, SR 368: This project consists of FM No. 450818-1-62-01 and is located along SR 368 for 3.469 miles from just east of SR 390 to just west of SR 75 (US 231). Also included is the milling and resurfacing of SR 327 (Lisenby Avenue) from SR 368 (23rd Street) to SR 390 for 0.545 miles and SR 390 (Beck Avenue) for 0.582 miles from just north of SR 30 (15th Street) to just south of SR 368.
The project scope includes:
• Milling and resurfacing of the travel lanes, median lanes, turn lanes, side streets, and paved shoulders;
• Pedestrian/ADA improvements;
• Sidewalk construction;
• Travel lane and driveway reconstruction;
• Signalization;
• Lighting; and
• Signage and pavement markings.
Although these are two individual projects, they will be let under one construction contract with 679 contract days to complete both and are planned to let through the FDOT’s Central Office in February 2026. It is expected that the NTP will be issued in April 2026.
2) UNDERSTANDING OF PROJECT ISSUES
The Volkert Team is familiar with the project location, stakeholders, and environment; has reviewed the project plans/specifications;
and has developed an understanding of the scope of work and potential challenges. To gain a full understanding of the scope of work and project issues, the Volkert Team has made multiple site visits; reviewed the plans; and met with Kevin Fussell, the FDOT
D3 Assistant Construction Engineer, Engineer-of-Record (EOR), and the Technical Review Committee (TRC). We will ensure that our staff and the contractor's staff remain well-informed and promote a collaborative environment to proactively address potential problems to deliver this project timely and efficiently.
Volkert will monitor the contractor’s operations to ensure the project is built in conformance with the plans and specifications for the project. We will determine if the contractor's quality control (QC) is adequate and will take steps to ensure compliance throughout the duration of the contract. The FDOT PM, Linda Melvin, will be integrated into our process of issue assessment, analysis, and development of engineered solution recommendations to allow for Departmental review and mutual approval.
Roadway Reconstruction Areas Issue: On both projects, there are several locations along the roadway that are failing and are not noted in the plans. These locations need to be reconstructed.
Plan of Action: These additional areas for reconstruction are located at the following locations:
SR 75 || 450810-1-52-01
• Sta 28+55 LT in the outside lane at an existing structure
• Sta 43+20 to 45+20 RT in the outside lane in the right wheel path
SR 368 || 450818-1-52-01
• Sta 82+00 LT
• Sta 157+20 RT (Wilson Avenue) Our team proposes utilizing the Travel Lane Reconstruction detail, as noted on Plan Sheet 3 of FPN 450810-1-52-01, for remediation of these areas. Our team will present a detailed estimate of the locations, quantities, and a cost estimate for review by the EOR and Department.
Driveway Reconstruction at Sta 65+70 RT Issue: At this driveway to be reconstructed just north of SR 77, there is existing curb and gutter on the south side of the driveway that is not shown to be removed and replaced as part of the driveway reconstruction. Also, there is shoulder buildup on both sides of the driveway preventing adequate stormwater runoff.
Plan of Action: After our field review of this location, the Volkert Team recommends the removal and replacement of the existing curb and gutter and the establishment of a new gutter grade line that will allow for the needed removal of stormwater water from the driveway. Also, it is also recommended that the adjacent shoulder areas of the driveway be graded to allow for the flow of water away from the driveway area.
Shoulder Buildup Issue: On SR 75, along the right roadway shoulder from Sta.
65+20 RT to the end of the project at Sta. 170+68 (excluding Sta.
147+71 RT to Sta. 150+34 at Game Farm Road), over time significant shoulder buildup has restricted stormwater drainage from the roadway. This condition causes water to back up into the travel lanes during periods of heavy rainfall, creating a hydroplaning hazard for vehicles.
Plan of Action: It is recommended that the shoulders along the noted areas be reworked/graded to allow for the flow of water from the roadway and the excess material removed from the project.
Southbound Travel Lane Correction Issue: The SR 75 plan sheet 30 shows plans to remove and replace the curb and gutter and place asphalt overbuild in the outside left lane. However, it does not appear that the limits noted in the plans are sufficient enough to fully correct roadway sinking.
Plan of Action: After our field review, the Volkert Team has noted that this area should be extended 110 feet south to Sta 145+00 LT to allow for additional area in failure. We also noted that the asphalt overbuild area for the southbound travel lane correction should be extended 110 feet south to Sta145+00 LT to allow for the additional area that appears to also be failing. Another option for correction of this area would be to reconstruct the area per the Travel Lane Reconstruction Typical Section detail on plan sheet 3 in lieu of the asphalt overbuild. This area could be susceptible to further failure if the underlying issue that is causing the roadway to sink is not corrected.
Additional Sidewalk Reconstruction Areas Issue: During our project site reviews, our team discovered additional busted and/or broken sidewalk segments not shown in the plans and as compared to the Estimated Quantities Report.
Plan of Action: Our team will coordinate with the EOR’s for each project and FDOT to quantify any additional areas needing removal and replacement to correct ADA accessibility issues. Our team will present a detailed estimate of the locations, quantities, and a cost estimate for review by the EOR and Department.
Inlet Top Replacement Issue: At the northeast corner of SR 368 and Winona Drive, there is an inlet top that is damaged. The inlet opening is restricted, which is causing the water flow to be minimal.
Plan of Action: The Volkert Team recommends removal and replacement of the inlet top to allow for proper stormwater flow into the drainage system at this location.
Additional Milling Depth at Frankford and Jenks Avenue Issue: At the intersection of SR 368 at Frankford Avenue and Jenks Avenue, rutting in a greater capacity than the rest of the project is present.
Plan of Action: The mainline Typical Section No. 1 on plan sheet 3 for SR 368 indicates to mill two inches and replace with two inches of FC-12.5 with High Polymer; however, additonal depth milling would be a benefit at these two intersections to remove the rutting.
The Volkert Team recommends increasing the milling depth and resurfacing thickness through these intersections to improve rideability and reduce the probablity of early rutting of the new asphalt.
Adjustment of Existing Manholes and Water Valves Issue: During our field review for these projects, it was noted that there are several utility manholes and water valves that will need to be adjusted prior to milling and resurfacing activities that are not shown in the plans.
Plan of Action: The Volkert Team will assess both projects and note the unidentified utilities that will need to be adjusted. We recommend meeting with the EOR to discuss the possible omissions and, if warranted, issue a plan revision with new quantities. If a plan revision is not feasible, the Volkert Team will track the overruns and represent them on the final as-built plans.
Underground Irrigation Sytems Issue: During our field review for the SR 368 project, it was noted that underground irrigation systems are present throughout the project limits at various locations. This typically causes the outside lanes to be saturated at night affecting paving operations.
Plan of Action: The Volkert Team will proactively interact with local property owners ahead of resurfacing operations to schedule system shutoff periods so that asphalt operations will not be impacted.
Utility Strip Buildup Issue: Most of the utility strip areas between the curb and sidewalk have a build up of sediment and grass over time. This does not allow the water to runoff the sidewalk as shown in Standard Plans Index 522-001, Sheet 1 of 2.
Plan of Action: It is recommended that the utility strip areas be graded and sodded, per the Standard Index, to eliminate the ponding of water during periods of rainfall, allowing for water to flow from the sidewalk area into the curb and gutter.
Note: Railroad Coordination – Per the project plans, work will occur within the existing railroad right-of-way (R/W) on SR 75 and SR 390 (Beck Avenue). During the preconstruction meeting, it will be discussed that the railroad will need advanced notice of the construction schedule and that railroad flaggers will need to be onsite anytime work activities occupy or have potential to impact railroad R/W or tracks. All railroad activities will be coordinated through the D3 Rail Administrator, Ray Corbitt, in advance of any work commencing within the vicinity of the railroad. Also, it will be noted that no work can commence prior to the contractor procuring railroad protective liability insurance coverage (as noted in Standard Specification Section 7-13.3, Insurance Required for Construction at Railroads) before accessing railroad R/W and that coverage complies with that noted in the standard specifications and acceptable to the railroad company.
Note: Maintenance of Traffic (MOT) – Volkert places the highest priority on SAFETY while managing our projects. Our inspectors will assess MOT daily for compliance with the contract plans, specifications, and standard plans. Our inspectors will be specifically tasked with MOT assessment, in addition to and in coordination with their other duties, allowing Volkert inspectors to proactively collaborate with the contractor to promote safe practices and identify issues before they occur to create a SAFETY-FIRST ENVIRONMENT. The Volkert Team takes pride in proactively collaborating with the contractors to identify any issues or concerns.
MOT and safety must be of the highest priorities, and we will constantly review the MOT setup to make necessary improvements to signage, striping, and traffic control device layout to improve operations. The Volkert Team will monitor the Lane Closure Information System (LCIS) daily to ensure that the lane closures are being initiated and updated daily.
Note: Evacuation Routes – SR 75 and SR 368 are both primary corridors through Panama City. In the event of a hurricane or named storm landfall, a large volume of traffic is anticipated to utilize these roadways. It will be of the utmost importance that all lanes are reopened for normal traffic within 12 hours of a Department evacuation notice. Volkert will be ready to instruct the contractor to implement its hurricane evacuation plan in a timely manner upon receiving notification from FDOT.
Note: Environmental Compliance – Erosion control and environmental protection are critical for any project, and these areas must be protected during construction. On SR 327 (Lisenby Avenue), there are habitats in the area from Sta 111+26.91 to Sta 131+95.98 for the Panama City Crayfish, which is a federally listed species. This area will be protected by installing sediment barrier as outlined in the plans. Volkert understands the importance of ensuring the contractor adheres to all plan, permit, and stormwater pollution prevention plan (SWPPP) conditions. Our proposed personnel have the required qualifications and experience to monitor wetland areas, conduct erosion measures, perform inspections routinely, and ensure the contractor submits all the required certifications within the allotted time frame. Volkert personnel will monitor the provisions of all related documentation, monitor permits, verify that the contractor is conducting inspections, preparing reports, and monitoring all stormwater pollution prevention and measures associated with the project, as required.
Note: SR 390 Construction – On the west end of SR 368 and north end of Beck Avenuve, the existing SR 390 project is currently under construction. PA Scott Prescott will coordinate with RS&H project personnel to ensure work activities for either project is not conflicted.
Note: HCA Hospital Access – Hospital and Emergency Room access is from SR 368, and it will be imperative not to disrupt the flow of traffic into and out of the hospital facility. Timely notice (minimum one week prior) of any lane closures, traffic shifts, and detours between State Avenue and Jenks Avenue will need to be related to Joel Leone, Chief Operating Officer (COO). It is recommended that a flagger be placed on this entrance/exit to facilitate the flow of traffic.
Note: Bayway Transit – Bayway Transit will remain operational and provide service for the duration of the project. Timely notice (minimum one week prior) of any lane closures, traffic shifts, pedestrian detours, bus re-routing, and bus stop relocations will need to be related to Lamar Hobbs, Transit PA. Vehicular and pedestrian access to bus stops must be always maintained during the duration of the projects.
Note: Public Information and Community Awareness – Our team understands the importance of community awareness and the impacts that construction can have on businesses and residents.
These projects have several locations where sidewalk improvements will be constructed near businesses and private residences, which can cause concerns. Project personnel will interact with those impacted locally to ensure they understand what is being constructed and when they can anticipate any impacts. It is our experience that communication with businesses and residents in the project area goes a long way to understanding concerns of the public and mitigate any issues before they arise.
Coordinating day and nighttime work schedules with businesses and impacts to driveways will be particularly critical for both projects. To provide the community outreach needed for these projects, Volkert has partnered with ABC, who will provide public information and community outreach services for the projects. With the Group 26-F projects being in an urban setting, community awareness of construction activities and special events are critical.
SR 75, SR 368, Beck Avenue, and Lisenby Avenue are integral corridors for commuters. Resurfacing operations will improve the roadways for the thousands of commuters that travel these roads daily. Therefore, key stakeholders (businesses and residents) must have a clear understanding of the project scope and how these projects will affect their community. FDOT’s goal is to connect and communicate impacts that the project will have on the public. ABC will coordinate weekly with the FDOT Public Information Office (PIO) to ensure they are kept up to date with construction activities and lane closures, as well as assist with outreach efforts, such as gathering and updating contact information of interested parties and assisting with dissemination of notices. In coordination with the Volkert Team and the D3 PIO, ABC will develop a Community Awareness Plan (CAP) and administer a public information program to engage key stakeholders before and during construction. Proposed public information services that ABC will provide and track include:
• Coordinating direct mail notifications for geographic stakeholders,
• Preparing weekly news releases for area media and elected/appointed public officials,
• Managing social media platforms and producing content for the
D3 website (www.NWFLRoads.com) and social media accounts, and
• Responding promptly (i.e., telephone calls, written correspondence, emails, and any other response formats) to any/all stakeholder inquiries.
These public information activities will demonstrate the Department's commitment to the community and empower stakeholders to make informed travel decisions during construction.
3) QUALITY CONTROL
Field QC: With the major focus of work being milling and resurfacing, PA Scott Prescott will collaborate closely with the contractor; FDOT D3 Pavement Materials Engineer, Hays
Griffin, PE; and his team to ensure all work complies with the plans and specifications. Our inspectors will verify all work is compliant with the required QC inspections checklists. Scott will lead the prepave meeting; identify and resolve any issues; note expectations regarding contract compliance; and discuss the daily paving procedures, from the setup of MOT to take down at the end of the workday. While milling and resurfacing activities are ongoing, our inspectors will:
Verify cross-slopes before and after the milling operations and recommend corrections, as needed.
Check milling depths and correct, if necessary.
Ensure proper application of tack coat.
Verify the new asphalt mat thickness/check the spread rate as asphalt operations commence/continue.
Discuss all non-compliance issues with the contractor.
Project QC will also extend to all items of work, such as drainage, concrete, signage, and thermoplastic, to ensure that all materials meet APL and Buy America, Build America (BABA) requirements and that material certifications are obtained and are in accordance with the project specifications.
CEI Quality Assurance (QA): Volkert’s objective is to improve daily in everything we do, and we are committed to providing quality services. Volkert’s commitment to quality will also extend to our subcontractors, their employees, and their work. Teresa Driskell, PE, Vice President over Volkert’s Florida CEI group, will serve as our team’s QA Manager. In this role, Teresa will have full authority to conduct QA audits and reviews and will recommend modifications to policies, procedures, and/or personnel to ensure compliance.
Proven Quality Assurance Audit (QAA) Program: Within 30 days of receiving the notice of award, Volkert will submit a project-specific QAA program to the District’s PM for review and acceptance. The plan will describe the responsibilities of each position on the project, the criteria for measuring individual and team performance, project safety measures, and quality of project deliverables. The plan will also highlight methods used to evaluate fulfillment of the scope requirements. Volkert’s QAAs involve interviewing key staff, contractor personnel, and FDOT employees.
QA Reviews and Contract Compliance: The QA team will verify contract compliance by completing all checklists. All non-compliant answers will be explained in detail. If discrepancies are found, the discrepancy and resolution sheet will include details discussed with the PA. Resolutions will include an action plan to correct any discrepancy. Teresa will review each discrepancy and resolution sheet from the audit to provide certification of the proposed course of action and will be responsible for verifying that the inspection as a whole and all documentation is performed in accordance with the construction plans, FDOT Specifications, and FDOT Design Standards and Procedures. The results of the QA Reviews will be shared with the FDOT PM, Linda Melvin, in a timely manner.

11/25
Pursuant to Section 287.055(5)(a), Florida Statutes, for any lump-sum or cost-plus-a-fixed fee professional services contract over the threshold amount provided in Section 287.017, Florida Statutes for CATEGORY FOUR, the Department of Transportation (Department) requires the Consultant to execute this certificate and include it with the submittal of the Technical Proposal, or as prescribed in the contract advertisement.
The Consultant hereby certifies, covenants, and warrants that wage rates and other factual unit costs supporting the compensation for this project’s agreement are accurate, complete, and current at the time of contracting.
The Consultant further agrees that the original agreement price and any additions thereto shall be adjusted to exclude any significant sums by which the Department determines the agreement price was increased due to inaccurate, incomplete, or noncurrent wage rates and other factual unit costs. All such agreement adjustments shall be made within (1) year following the end of the contract. For purposes of this certificate, the end of the agreement shall be deemed to be the date of final billing or acceptance of the work by the Department, whichever is later.

Instructions for Certification
Instructions for Certification - Lower Tier Participants:
(Applicable to all subcontracts, purchase orders and other lower tier transactions requiring prior FHWA approval or estimated to cost $25,000 or more - 2 CFR Parts 180 and 1200)
a. By signing and submitting this proposal, the prospective lower tier is providing the certification set out below.
b. The certification in this clause is a material representation of fact upon which reliance was placed when this transaction was entered into. If it is later determined that the prospective lower tier participant knowingly rendered an erroneous certification, in addition to other remedies available to the Federal Government, the department, or agency with which this transaction originated may pursue available remedies, including suspension and/or debarment.
c. The prospective lower tier participant shall provide immediate written notice to the person to which this proposal is submitted if at any time the prospective lower tier participant learns that its certification was erroneous by reason of changed circumstances.
d. The terms "covered transaction," "debarred," "suspended," "ineligible," "participant," "person," "principal," and "voluntarily excluded," as used in this clause, are defined in 2 CFR Parts 180 and 1200. You may contact the person to which this proposal is submitted for assistance in obtaining a copy of those regulations. “First Tier Covered Transactions” refers to any covered transaction between a grantee or subgrantee of Federal funds and a participant (such as the prime or general contract). “Lower Tier Covered Transactions” refers to any covered transaction under a First Tier Covered Transaction (such as subcontracts).
“First Tier Participant” refers to the participant who has entered into a covered transaction with a grantee or subgrantee of Federal funds (such as the prime or general contractor). “Lower Tier Participant” refers any participant who has entered into a covered transaction with a First Tier Participant or other Lower Tier Participants (such as subcontractors and suppliers).
e. The prospective lower tier participant agrees by submitting this proposal that, should the proposed covered transaction be entered into, it shall not knowingly enter into any lower tier covered transaction with a person who is debarred, suspended, declared ineligible, or voluntarily excluded from participation in this covered transaction, unless authorized by the department or agency with which this transaction originated.
f. The prospective lower tier participant further agrees by submitting this proposal that it will include this clause titled "Certification Regarding Debarment, Suspension, Ineligibility and Voluntary Exclusion-Lower Tier Covered Transaction," without modification, in all lower tier covered transactions and in all solicitations for lower tier covered transactions exceeding the $25,000 threshold.
g. A participant in a covered transaction may rely upon a certification of a prospective participant in a lower tier covered transaction that is not debarred, suspended, ineligible, or voluntarily excluded from the covered transaction, unless it knows that the certification is erroneous. A participant is responsible for ensuring that its principals are not suspended, debarred, or otherwise ineligible to participate in covered transactions. To verify the eligibility of its principals, as well as the eligibility of any lower tier prospective participants, each participant may, but is not required to, check the Excluded Parties List System website (https://www.epls.gov/), which is compiled by the General Services Administration.
h. Nothing contained in the foregoing shall be construed to require establishment of a system of records in order to render in good faith the certification required by this clause. The knowledge and information of participant is not required to exceed that which is normally possessed by a prudent person in the ordinary course of business dealings.
i. Except for transactions authorized under paragraph e of these instructions, if a participant in a covered transaction knowingly enters into a lower tier covered transaction with a person who is suspended, debarred, ineligible, or voluntarily excluded from participation in this transaction, in addition to other remedies available to the Federal Government, the department or agency with which this transaction originated may pursue available remedies, including suspension and/or debarment.
