About this file
This document is an Amendment to a Standard Professional Services Agreement between the State of Florida Department of Transportation (FDOT) and Environmental and Geotechnical Specialists, Inc. (EGS), located in District Two Professional Services Office in Lake City, Florida. The amendment involves the original agreement number CAY15, dated 7/28/2025, and is specifically Amendment Number 1 executed on 08/25/2025.
The amendment adds the Florida Highway Patrol as a subconsultant to the existing agreement for Materials Services, specifically for Consultant PE Lab Testing Materials/Geotechnical services. The subconsultant will be compensated at a rate of $75.00 per hour for MOT Off-Duty Law Officer with Vehicle Home services, with no additional multipliers applied. The services will be paid based on approved time incurred during the billing period, and the amendment updates Table 6 of Exhibit "B" in the original agreement to reflect these new rates and services.
