PROJECT UNDERSTANDING: Johnson, Mirmiran & Thompson (JMT) is excited for the opportunity to provide design services for the Resurfacing, Restoration, and Rehabilitation (3R) along SR-5/US-1 from S of SR-814/Atlantic Blvd (MP 8.756) to N of SR-834/Sample Rd (MP 11.863), a Federally-funded 3.1-mile project in the Cities of Pompano Beach (S of NE 24 St) and Lighthouse Point (N of NE 24 St), Broward County (County). Project Overview: SR-5 is a 6-lane north-south “urban principal arterial - other”, with a design/posted speed of 45/45 MPH and AADT of 42,000 to 51,000. The Project Level Context Classification (PLCC) is C4-Urban General. Land use is generally commercial adjacent to SR-5 with residential neighborhoods immediately behind the fronting commercial.

KEY PROJECT ISSUES: Roadway/Typical Section: Existing: In each direction, the last resurfacing project (227391-1; FY01; metric) established three 11’ to 12’ travel lanes, a 4’ bike lane (except from Atlantic Blvd to NE 4 St), Type F curb & gutter, and a 6’ to 10’ sidewalk divided by a 19.5’ to 28’ landscaped Type F curb median. Proposed: Per the 4/2022 memo from D4 OMD we anticipate using a target speed of 40 MPH and reducing the posted speed to 40 MPH. To accommodate buffered/wider bike lanes, we will coordinate with the DDE to restripe the typical section, as described below. JMT recommends making the Sample Rd intersection a milling exception as it was recently resurfaced under FM 434695-1. Bike Lanes: Widen existing bike lanes as follows: NE 18 to Copans Rd – reduce existing 11.5’ lanes to 11’ to provide 5.5’ bike lanes. NE 4 St to Sample Rd - reduce existing 12’ lanes to 11’ to provide 7’ buffered bike lanes.

Atlantic Blvd to NE 4 St - Bike lanes are absent and cannot be accommodated due to restricted pavement width/ROW; therefore, a DV for lack of bicycle facilities will be required for this segment. Pavement Design: The pavement design for milling and resurfacing will specify 1.25” minimum lifts for FC to avoid density concerns and potential shortening of the pavement life associated with 1” lifts. A pavement design for reconstruction is also anticipated to address subbase failures caused by leaking drainage pipes. Drainage: Several sections of roadway within the project limits have sunken and have been patched. We recommend inspecting the existing system and/or requesting maintenance records to identify pipe deterioration and collaborating with the D4 Drainage office to determine locations

375-030-22

PROCUREMENT

requiring pipe lining or replacement. To prevent further settling, we recommend either full depth excavation to replace the subbase with clean soil that meets FDOT specifications (as designed by JMT on FM 437834-1) or inserting flowable fill into the soil (requested by Lighthouse Point PWD, as it has been used by their city and helps reduce construction duration). There is a documented flooding complaint at the intersection of SR-5 and NE 2 St. As-builts indicate that the intersection was intended to be a high point in the drainage basin. Surveyed elevations will be used to determine if the current intersection elevation deviates significantly from its original design.

The proposed intersection elevation can be altered slightly while resurfacing to address the flooding issue. Additionally, a spread analysis and hydraulic analysis of the existing pipes will be performed to determine if more inlets and/or larger pipes are required. This project will be exempt from requiring an ERP, a SWPPP will be required if the area of full depth reconstruction exceeds one acre. We will prepare the Mini-Drip Memorandum, PIF and PRM during design. Pedestrians/ADA/ Transit: Substandard curb ramps and sidewalks will be reconstructed to meet ADA and detectable warning requirements. JMT will plan to prepare exhibits for the removal of several abandoned driveways. BCT Route 10 runs along the corridor; several stops will be upgrading to provide proper boarding/alighting areas. JMT has identified several intersections where Pedestrian Hybrid Beacons (PHBs) may be warranted to address ped issues, including multiple fatalities along the corridor, in line with the FDOT Target Zero initiative. Based on injury crashes and fatalities, potential PHB locations are (1) between 6th St and 10th St, which runs along the park/amphitheater/sports complex; (2) NE 25 St; and (3) NE 31 St. The usage/locations will be discussed with Traffic Ops prior to incorporating into the scope. Adjacent Projects: There are several overlapping/adjacent FDOT projects (FM Nos 445625-1 Intersection Lighting; 447672-1 SR-844 3R; 440086-2; and 439891-5 SR-869 TSM&O Smart Work Zone) and County “Penny for Transportation” projects (CPD03 Atlantic Blvd Design Study; A17 Sample Rd Adaptive Signals; and F18: Hillsboro F/O Network). We will coordinate construction schedules and harmonize the project limits with adjacent projects as necessary. Design Variations (DVs): Anticipated DVs include intersection sight distance, cross slope (outside 3R range), lateral offset (signals in control zones) and unobstructed sidewalk at some signal facilities. S&PM: All substandard signs will be upgraded, and pavement markings replaced, including school crossings. Signalized intersection crosswalks will be upgraded to high emphasis, eliminating the existing patterned pavement. We will coordinate with Pompano Beach regarding the Airpark Shared Use Path (SUP) parallel to SB SR-5; at side street crossings, the SUP markings/path terminals should be harmonized with the FDOT sidewalk/curb ramps, which will likely require an off-system agreement with the City. Missing “Signal Ahead” signs will be added where required, and existing signs with poor reflectivity will be replaced. R1-5b signs will be added for all right turn crosswalk approaches at the signals, and green markings added to bicycle keyhole lanes. Arterial wrong way driving countermeasures will be incorporated in the signing and pavement marking design in accordance with FDM 230.4.3. “Bike lane end” signage will be added SB approaching NE 4 St. Signalization & Misc.

Structures: There are 12 signalized intersections, all with video detection. NE 24 St has strain pole signal supports; the rest have painted mast arms. We noted 33 deficient pedestrian signals and 60 deficient pedestrian detectors to be brought to current standards. The NW mast arm at NE 14 St is damaged and will be evaluated to document its condition and provide repair recommendations. Structural evaluations will also be performed for mast arms where additional loading has been recommended due to the addition of vehicular signal heads (i.e., Copans and Sample) and signs. There are seven locations where sidewalk clear width is <48” min. There are also six signal supports that violate the 48” min. offset from the face of curb. Our roadway and sidewalk design will bring these areas into compliance. One tricky aspect of design will be to maintain the existing elevations around the mast arm bases while reconstructing the ped ramps to meet ADA. Survey should include elevations around all mast arm bases. Lead Pedestrian Intervals (LPI) were recommended for the intersections of Atlantic Blvd and Sample Rd. We will coordinate the LPIs with BCTED. We will also send BCTED a list of maintenance items for their attention, such as mast arms with peeling paint and the faded/broken internally illuminated street name signs. Low-hanging utility lines which block signal visibility or that rest on the signal supports at NE 2 St and at Sample Rd will be reported to the UAOs and their adjustment included in the UWSs. We will discuss upgrading the NE 24 St span wire to mast arms with Traffic Ops, as it is within the Mast Arm Boundary Map; it may also be done under a DW or CSC task to save 3R funds. Temp.

Traffic Control Plan (TTCP): A Level 1 TTCP will be developed in accordance with FDM, FDOT Standard Plans 102-600, and the SOS.

Pedestrian detour plan details will be provided per Index 102-660. We will also specify/quantify longitudinal channelizing devices (LCD) where sidewalk closures are needed and will coordinate with Pompano Beach to detour peds to the Airpark SUP. We will maintain access to driveways, side streets, and bus stops and be mindful of the school crossings. Specific information regarding lane closures, anticipated construction start dates for the TTCP will be identified and discussed in the Community Awareness Plan (CAP) and Public Workshop at a minimum. FAA Coord: Pompano Beach Airpark is adjacent to the corridor. Using the FAA’s Notice Criteria Tool, we determined that Notice to FAA is required. Lighting: Existing decorative ped lighting from Atlantic Blvd. to NE 2 St will remain. A combination of HPS and LED lighting exists throughout the corridor. Proposed lighting retrofit is anticipated at 10 signals. From NE 16 St to Copans Rd, corridor lighting is proposed on the east side. Poles will be placed at the back of sidewalk, within the R/W. Conflict poles or spread footings may be considered as needed. The HPS lighting in these limits (west side) will be upgraded to LED. FDOT lighting compensation to the local agencies is a set amount per luminaire per year; the Cities maintain the existing lighting and therefore it is imperative that we coordinate immediately with both the County and the Cities to ensure they accept LT1 at the beginning of the project, as the proposed lighting will require a metered service. Utilities: There are 14 UAOs within the project corridor. Key issues include early requests for utility test holes (SUE) if required and identifying conflicts between buried facilities and proposed features. We will develop conflict matrixes and assist the UAOs in developing Utility Work Schedules, and all contacts and exchange of files will be made in the PSEE

Utility Coord. Module. Landscaping: JMT will coordinate with D4 Maint. and the D4 District LA, Bann Williams, regarding existing median and sidewalk tree canopies that require trimming to improve visibility of regulatory/street name signs and improve lighting uniformity; tree roots requiring pruning and root barriers to address uplifted sidewalk flags; and MMOAs with the Cities. Lighthouse Point requested sleeving/protecting the 3” reuse irrigation line in the medians in areas of curb/pavement reconstruction, as this line was damaged by FM 434695-1 north of Sample Rd. Maintenance: We will provide a memo of issues we noted in the field that can be resolved by D4 Maint.

well before construction, such as cracked or uplifted sidewalk trip hazards, damaged inlet grates and tops, and overgrown trees.

PROJECT MANAGEMENT: JMT begins every contract by developing a Project Management Plan (PMP) to be adopted by the Team.

The PMP is a living document customized to the District’s modus operandi as related to scope, schedule, budget, responsibilities, QA/QC, communications, document management, technology/software, risk/change management, and safety. Our internal and external project coordination will employ MS Teams, file sharing with ProjectWise; timely distribution of meeting minutes; regular progress meetings with dated action items/responsible party; and workload planning with using a 30, 60, and 90-day schedule look-ahead. The JMT Team is familiar with the District’s procedures and applications including: Phase Submittals/Checklists; coordinating with the TREV committee for timely TSP and DV approval (JMT recently coordinated lowering posted speed on FM 447678-1 SR-838); NEXGEN plans using ORD; use of PSEE for schedule updates, DARs, utility coordination, submittals, and Design Documentation; properly formatted ERC responses; AASHTOWare for pay items; task-specific progress reports/invoices with payout curves; and web-based Spec Packages.

Public Involvement: Major stakeholders include both Cities, the County/MPO, Pompano Beach Airpark, and 8 nearby schools. A Level 2 CAP will be developed and updated throughout the project life. A Public Workshop and meetings with key stakeholders are anticipated.

The public meeting will have a hybrid option using virtual platforms (ex. “Go-To Meeting” or “Go-To Webinar”).

SCHEDULE & BUDGET: We will maintain positive float in our PSEE schedule via a clear understanding of the SOS and recognizing which critical activities can derail the schedule, such as wants or needs from the Cities or County (ex. wider sidewalks for shared paths requested in the MMSC) for items not covered by the 3R project (ex. decorative sidewalk features). We will meet early with stakeholders to inform them of the project’s scope and 3R funding limitations. If they wish to fund additional scope, we will provide a timeline of milestones that must be met. Milestones include a written declaration of the intent to enter into a Locally Funded Agreement (LFA), the date an executed LFA would be required, and the date that funds must be transferred to FDOT, mindful of: (1) time needed to draft the LFA, (2) legal reviews by both entities, and (3) placement on a City Commission meeting agenda for approval/signature. Other closely monitored elements include driveway closures, executed agreements (ex. UWHC, off-system, maintenance, JPAs, license agreements), right-of-way maint. maps, and proprietary products. We strive for early completion of critical path items and focus on staff/resource assessment using our firm’s workload forecasting software, understanding this project is a candidate for Advanced Production. Our goal is not just to meet Production, but to guarantee to Ms. Ou an organized and seamless journey from NTP to letting, where all milestones are met, and schedule activities are updated in PSEE as they occur (not just the 1st week of each month). LRE updates will be prepared at project submittals and for each Work Program Update Cycle (WPUC), along with the EQ Report.

QUALITY CONTROL: Our QA/QC Team includes constructability reviews by our CEI experts, incorporating “lessons learned”. We are registered with the Contact Management /E-Updates and Contact Mailer database to stay current with FDOT information/training. We coordinate directly with ERC reviewers to ensure comments are properly interpreted. We also develop a hyperlinked Excel table that allows simple navigation between the plans and ERC comments, providing Ms. Ou with an efficient tool for comment verification.

KEY STAFF/ PROVEN EXPERIENCE: Ms. Zhao, PE has 11 years of District 4 transportation design, management, and CEI experience.

At NTP, Ms. Zhao will be 85% available, enabling her to maintain the highest degree of thoroughness and quality in the management of this project. Ms. Zhao’s recent District 4 experience includes being Deputy PM/Lead Highway Engineer on: SR-5/US-1 3R from N.

of George Bush Blvd to the C-16 Canal (FM 437834-1, D4 PM: Thuc Le, PE) which had similar drainage and pavement concerns and required considerable municipal coordination; SR-5 bike lane restriping over the Roosevelt Bridge to provide 7’ buffered bike lanes (FM 438345-1, D4 PM: Henry Oaikhena); and SR-9336 3R from SW 192 Ave to SR-5 with off-system agreements for a new SUP (FM 445992- 1, D6 PM: Ivette Funtanellas; 305-470-5270). JMTs D4 experience includes SR-811/NE 4 Ave (431657-1, FTBA Best in Construction;

National Roadway Safety Award; PM H. Arrieta) and SR-5/US1 (229744-3, “complete street” project nominated for FICE “Outstanding Roadway Project”; PM J. Hughes). In addition to roadway, drainage and S&PM, JMT will provide any required survey and SUE under WG 8.1 and 8.2. The JMT Team includes Civil Works, Inc. (CWI) (DBE/SBE), for Signalization; VN Engineers, Inc.

(VN) (DBE), who has worked with Ms. Ou on projects 439841-1 and 229744-4, for lighting, utility coordination, and misc. structures, under-utilized WG 4.1.1; Quest (DBE/SBE) for Public Involvement;

Tierra South Florida, Inc. (TSF) (DBE) for Geotechnical; Miller Legg (ML) (SBE) for Landscaping; and GPI for Photogrammetry.

As a result, each shortlisted consultant is requested to provide a Technical Proposal, resumes, organization chart or staffing chart, and required certifications and forms.

Shortlisted Consultants:

CHA Consulting, Inc

CORE Engineering Group, LLC Johnson, Mirmiran & Thompson, Inc.

2. Scope of Consultant Services:

For a detailed description of the required consultant services, refer to Attachment A, Scope of Services.

4. Contract Restrictions & Proposed Method of Compensation:

A standard consultant contract will be executed with the selected consultant for this project. The standard contract terms are provided as Attachment B. The following additional information is furnished to the consultant firm for consideration:

4.1 Professional Liability Insurance Requirement: $500,000.00. For purposes of contracting, only Professional Liability Insurance from a company or companies authorized to do business in the State of Florida is acceptable. The selected prime consultant shall submit proof of Professional Liability Insurance coverage in the minimum threshold amount designated above, prior to contracting. The policy/ies and coverage described herein may be subject to a deductible. Pay all deductibles as required by the policy. The Department shall be notified in writing within ten days of any cancellation, notice of cancellation, lapse, renewal, or proposed change to any policy or coverage described herein. The Department’s approval or failure to disapprove any policy/ies, coverage, or ACORD Certificates shall not relieve or excuse any obligation to procure and maintain the insurance required herein, nor serve as a waiver of any rights or defenses the Department may have. No policy/ies or coverage described herein may contain or be subject to a Retention or a Self-Insured Retention. A commitment letter from a financial institution is also not acceptable for purposes of contracting.

4.2 Computer Usage: The selected Consultant firm may desire to use the Department's mainframe computer (not CADD) for project related tasks.

4.3 Subconsultant Services: Services assigned to subconsultants must be approved in advance by the Department. Subconsultants performing the standard types of work covered by Rule Chapter 14-75, F.A.C., must be technically prequalified with the Department or have an application for prequalification under review at the time that they are proposed.

Subconsultants with fees less than $500,000, and/or performing specialty services (non-standard types of work), a self-certified overhead statement may be accepted. These overhead statements must be sent to Central Office for review and approval.

Private attorney services may not be utilized on Department contracts without advance approval from the Attorney General’s Office. The advance approval may only be requested by Agency General Counsel (FDOT), and not the consultant firm. Since approval cannot be assumed, consultant firms shall refrain from proposing legal firms as part of the professional services consultant team in their procurement response and proposal documents. As referenced in Section 287.059(3), F.S.:

“An agency requesting approval for the use of private attorney services shall first offer to contract with the Department of Legal Affairs for such attorney services at a cost pursuant to mutual agreement. The Attorney General shall decide on a case-by-case basis to accept or decline to provide such attorney services as staffing, expertise, or other legal or economic considerations warrant. If the Attorney General declines to provide the requested attorney services, the Attorney General’s written approval shall include a statement that the private attorney services requested cannot be provided by the office of the Attorney General or that such private attorney services are cost-effective in the opinion of the Attorney General. The Attorney General shall not consider political affiliation in making such decision. The office of the Attorney General shall respond to the request of an agency for prior written approval within 10 working days after receiving such request. The Attorney General may request additional information necessary for evaluation of a request. The

Attorney General shall respond to the request within 10 working days after receipt of the requested information.”

When universities or colleges are needed to perform services, the http://www.fdot.gov/procurement/pdf/Rule%2014-75.pdf

Department’s Project Manager shall procure such services by means of either a University Master Agreement task work order, a governmental agency contract, or a competitive solicitation for contractual services, as appropriate (reference Section 287.057(21), F.S.). Universities proposed as subconsultants will not be added to the professional services contract.

4.4 DBE/Small Business Participation:

The Department encourages DBE and small business firms to compete for professional services projects, and also encourages all other firms to use DBE and small business consultants as subconsultants. However, use of DBE and small business subconsultants is not mandatory and no preference points will be given during the selection process for DBE and/or small business participation.

Consultants shall complete Form No. 375-030-83 to indicate commitments anticipated for the project.

Federal law requires states to maintain a database of all firms that are participating or attempting to participate in DOT-assisted contracts. To assist the Department in this endeavor, consultants should have already submitted their Bid

Opportunity List through the Department’s Equal Opportunity Compliance System found at the following link (https://fdotwp1.dot.state.fl.us/EqualOpportunityCompliance/Account.aspx/LogIn?

ReturnUrl=%2fEqualOpportunityCompliance). If you have not yet completed your bid opportunity list through this online application, please do so at this time.

Contractors and Consultants New Users Access Form. Please complete the form and submit it to EOOHelp@dot.state.fl.us.

Also, if you are awarded amendments to task work orders, the subconsultant commitments will also need to be reported after the respective invoice has been paid.

4.5 Method of Compensation:
