This document is an Amendment to a Standard Professional Services Agreement between the State of Florida Department of Transportation (FDOT) and Azimuth 360 Consulting Group, Inc., with K-Factor, Inc. as a subcontractor. The amendment is for Contract CAK11, specifically for Construction Engineering and Inspection (CEI) Services for the Savannas Preserve State Park Gap project, located in District Four (Fort Lauderdale, Florida).
