About this file

This is a Task Work Order for Professional Services issued by the State of Florida Department of Transportation (FDOT) to Clary, Inc., a consulting firm located in Jacksonville, FL. The document is specifically for a bridge monitoring survey of the Dames Point Bridge (Bridge Number 720518) under Financial Project ID 426245-1-32-12, with the survey to be conducted using US Survey Feet measurements.

The task work order authorizes Clary, Inc. to perform a comprehensive bridge survey with a total compensation limit of $15,943.00, to be paid on a lump sum basis according to percentage of completion. The survey services include: 1) determining elevations of four targets on North and South Tower Piers, 2) establishing Northing, Easting, and Elevation for targets on Tower and Rest Piers, and 3) measuring the bridge deck profile by taking elevations along girder edges at approximately 17.5' increments. The work must be completed by March 27, 2027, and requires specialized techniques such as performing the deck measurements during nighttime hours to maintain consistent bridge temperature and traffic loading conditions.

SCOPE OF SERVICES

1.0 DESCRIPTION

REVISED: February 7, 2013

Authorization Financial Project ID 426245-1-32-12, DAMES POINT BRIDGE, Bridge Number 720518. This is a US Survey Feet, bridge monitoring survey.

The limits of the survey are: (See Figure No. 1)

Settlement of Tower Piers:

Determine the elevation of four (4) targets (A1-D1) on both the North and

South Tower Piers. (See Figure No. 2)

Verticality of Tower and Rest Piers:

Determine the Northing, Easting and Elevation of three (3) targets on each side of the two (2) Tower Piers and two (2) targets on each side of the two

(2) Rest Piers. (See Figure No. 10A and 10B)

Measuring Profile Grade of Bridge Deck: N/A

Take elevations along the edge of the girders both left and right, as marked by nails, at approximately 17.5’ increments.

2.0 SERVICES

For this authorization, the consultant shall provide all survey services necessary to obtain horizontal values in State Plane Coordinates NAD 83/90 and vertical values in NGVD 29 on the above mentioned bridge at the specific locations requested. These services shall include, but not be limited to the following:

A. Survey

1. Settlement of Tower Piers: (See Figure No. 2) Utilizing existing bench marks (See Figure No.’s 5-8) angles and distances will be turned to each target (See Figure No. 3) and recorded in a Field

Book. (See Figure No. 9) From the recorded field notes, trigonometric elevations for each target (NGVD 29) will be obtained and added to the provided table. (See Figure No. 4)

2. Verticality of Tower and Rest Piers: (See Figure No. 10A and 10B)

Utilizing existing points with an established Northing, Easting and

Elevation (See Figure No.’s 14-21) angles and distances will be turned to each target and or prism (See Figure No. 12) and recorded in a Field Book. (See Figure No. 22) From the recorded field notes a position in State Plane Coordinates (NAD 83/90) and

Vertically (NGVD 29) will be established and added to the provided table. (See Figure No. 13A and 13B) The centerline of the bridge will be established by determining the center of the

North and South Pedestal and a bearing for the centerline will be fixed in State Plane (NAD 83/90). The verticality alignment, being called vertical baseline “X”, will be a bearing at right angles to the centerline bearing of the bridge. (See Figure No. 11) To establish the offsets for the targets and single prism on top of the towers

(Note: all single prisms are set two (2’) north of center) the coordinate value for target T1 for each pier will be used along with the verticality alignment bearing being called vertical baseline

“X”. (See Figure No. 10A and 10B) Extending the verticality alignment bearing, vertical baseline “X”, through the coordinate for target T1, which will occupy a vertical plane, an offset can be calculated and added to the provided table (See Figure No. 13A and 13B) for target T2 and the single prism. Calculations will be done separately for each pier face.

Note: A work boat and motor will be required for this portion of the survey.

3. Measuring the Profile Grade of the Bridge Deck: (See Figure No.

22) Elevations are taken along the edge of the girders on both the

East and West sides of the bridge, at approximately 17.5’ increments, just behind the edge barriers. The exact location of each point to be surveyed is marked by a nail that was set during construction, and painted corresponding alpha numeric name

(Note: The names are to be repainted with white paint, during this phase of work). To establish elevations on each nail a level loop will be performed from the northwest corner of the north end span from a bench mark at the base of the bridge. (See Figure No. 24)

The level loop will run south along the west side of the bridge and then north along the east side of the bridge checking into the beginning benchmark. The nails in the girders, may be side shots, but must be adjusted with the level loop (Note: It is recommended that a Dumpy Level be used for this task). The adjusted elevations will be added to the provided table and the differences between the previous and current survey will be calculated. (See Figure No. 25)

Note: The work for this portion of the survey should be done during the hours of darkness, to maintain consistent bridge temperature and traffic loading.

4. The project surveyor shall certify by a signed and sealed letter the survey information was collected according to the FDOT Location

Survey Manual (Topic No.’s 550-030-001, 550-030-030, 550-030-

100) and meet or exceed the Minimum Technical Standards for

Land Surveyors as set forth in the Florida Administrative Code

Chapter 5J-17, pursuant to Florida Statute 472-027 and any other special instructions that may apply.
