About this file

This document is an Amendment to a Standard Professional Services Agreement between the Florida Department of Transportation (FDOT) District Five and Tetra Tech, Inc., with additional sub-consultants including Dannick Engineering Consultants, Quest Corporation of America, Traffic Engineering Data Solutions, and Maser Consulting P.A. The amendment is for Financial Project No. 441020-1-32-01, specifically concerning the SR 519 (Fiske Boulevard) project in Brevard County, spanning from the I-95 northbound ramps/Barnes Boulevard to SR 520 (King Street).

The amendment increases the total contract amount by $32,866.00, bringing the total maximum limiting amount to $2,205,135.00. The additional funds and scope are to incorporate improvements requested by the City of Cocoa for Provost Park, including adding curb and gutter along southbound lanes, relocating driveways, and reconfiguring parking areas. Additionally, FDOT's Brevard Operations requested reconstruction of pavement sections experiencing base failure. The amendment requires Tetra Tech to develop new pavement designs, typical sections, drainage analyses, traffic control plans, and update various project documentation. The work is to be completed by June 1, 2020, within the existing project schedule, with Tetra Tech receiving the bulk of the additional funding ($32,866.00) for these expanded design services.

SCOPE OF SERVICES

SR 519 (Fiske Blvd.) from I-95 NB Ramp/Barnes Blvd. to SR 520 (King St.)

Financial Project ID No. 441020-1-32-01

1.0 BACKGROUND

Tetra Tech (CONSULTANT) was originally retained by the Florida Department of

Transportation District Five (DEPARTMENT) under Contract CA229 to provide construction plans for the resurfacing of SR 519 (Fiske Boulevard) from the I-95 northbound ramps/Barnes

Boulevard to SR 520 (King Street) in Brevard County (FPID 441020-1-32-01).

At a local agency coordination meeting, the City of Cocoa stated that they were planning improvements to Provost Park, which is at the north end of the SR 519 project limits. The improvements included adding curb and gutter along the southbound lanes, relocating and consolidating the driveways, and reconfiguring the parking areas. The City of Cocoa requested that the curb and gutter and driveway modifications be included in the SR 519 project to enhance the safety of pedestrians since the park improvements are not funded and will not be constructed prior to the SR 519 project.

In addition, FDOT’s Brevard Operations office noted that they have been repairing sections of the SR 519 pavement within the project limits on a regular basis as a result of base failure. They requested that these pavement sections be reconstructed as part of this project instead of being resurfaced.

2.0 PURPOSE

The purpose of this Supplemental Amendment is to provide authorization for additional design services to incorporate the proposed improvements to Provost Park and reconstruction of the pavement that has experienced base failure into the construction plans for SR 519. This Scope of

Work provides additional compensation to the CONSULTANT’s design contract.

3.0 SERVICES TO BE PERFORMED

A. Roadway

1. Develop a pavement design for reconstruction of SR 519 in the following areas that have experienced base failure, and revise the Pavement Design Report accordingly:

Exhibit "A"

3. Develop an additional typical section showing curb and gutter along the SR 519 southbound travel lanes.

4. Perform design analysis to develop a gutter grade, ensuring that the minimum

0.3% grade is provided, and tie the proposed back-of-sidewalk elevation to the existing ground.

5. Revise the design file to reflect the reconstruction areas, include curb and gutter and a bike lane, to relocate the driveway to Provost Park, and to replace the existing sidewalk with a 6-ft sidewalk.

4. Develop additional drainage structure cross sections and update the Summary of

Drainage Structures

5. Incorporate the proposed drainage structures into the Erosion Control Plan Sheets, and SWPPP.
