About this file

This document is an Amendment to a Standard Professional Services Agreement between the State of Florida Department of Transportation (FDOT) and Metric Engineering Inc. The amendment is specifically for District Five Professional Services Office, located in DeLand, Florida, and modifies the original agreement (C9W03) executed on 8/28/2017. The amendment is dated 4/28/2020 and is related to Pre-Event No. 3 Natural Disaster Monitoring/Construction Engineering & Inspection services.

The amendment outlines detailed documentation requirements for disaster monitoring and construction engineering and inspection (CEI) services. Key requirements include submitting comprehensive invoicing packages with load tickets, photographs, truck certifications, permits, and Excel summary sheets. The consultant must track debris through its entire lifecycle, document specific details on load tickets (such as GPS coordinates, contractor name, and debris type), and ensure all photographs and documentation meet strict quality standards. The amendment emphasizes precise record-keeping and documentation to support federal reimbursement, with specific guidelines on naming conventions, invoice packaging, and load ticket components.
