About this file
This document is an Amendment (Number 4) to a Standard Professional Services Agreement between the State of Florida Department of Transportation (FDOT) and Asha Planning Consultantcy, Inc. The original agreement was executed on 3/28/2016, with this amendment dated 12/13/2018, originating from FDOT's District Five Professional Services Office located in Deland, Florida.
The agreement covers a Continuing Services Contract for Modal Office Outreach Support, identified by FM Number 437512-1-12-01. The amendment primarily adds financial performance provisions, specifically outlining consequences for unsatisfactory service delivery. These include a requirement for the consultant to provide a corrective action plan within five days of being notified of performance deficiencies, and potential assessment of a 10% retainage on invoice amounts if performance issues are not adequately addressed. The contract allows for retainage to be returned if deficiencies are subsequently resolved, or forfeited at the end of the agreement period if unresolved.
