About this file
This is Amendment No. 1 to DEP Contract No. LE705, effective November 17, 2014, between the Florida Department of Environmental Protection (DEP) and Perma-Fix of Florida, Inc. The amendment modifies the terms and conditions of the original indefinite delivery contract between these two parties.
The amendment revises four key contract sections: invoicing and payment procedures, payment methods, notice delivery addresses, and contract manager designations. Under the revised invoicing requirements, the contractor must submit itemized invoices in accordance with authorized budget categories listed in Attachment D, using the Contractual Services Invoice Form. Invoices for waste transportation or disposal must include original or photocopied manifest documentation, and invoices for analytical fees must include photocopied lab reports. The contractor must also complete the Subcontractor Utilization Report Form with each invoice. The amendment permits fixed-price negotiations on a site or per-occurrence basis through written work orders signed by the DEP Project Manager. For projects anticipated to exceed $250,000 or require more than one month to complete, the Department reserves the right to request proposals. Payment procedures were modified to allow invoicing for completed work portions in their entirety and, for projects exceeding six months, monthly invoicing with pre- and post-audit submission requirements. The final invoice must be submitted within thirty days of contract completion. Contract management responsibilities were reassigned to Melissa Woehle and Jonathan Lara at the Department, with Glenn Byer serving as the Contractor's manager.
