Contract Summary
This document is a contract amendment between the Florida Department of Environmental Protection (DEP) and EQ Florida, Inc., dated August 21, 2008, amending the original contract executed on June 26, 2006. Amendment No. 2 modifies DEP Contract No. LE619 to authorize funding upon execution of Work Authorization Forms rather than through direct encumbering of funds at contract inception. The contract manager is Robyn Gedeon, and the contract was approved by DEP legal counsel and the DEP Contracts Administrator.
The contract is for abandon drum removal services under a cost reimbursement payment method. Work is authorized and funded through individual Work Authorization Forms (WAF) issued by the Department, with authorized representatives including Emergency Response Managers and Emergency Response Specialists for the Department, and Site Supervisors or higher-level positions for the contractor. The contractor is prohibited from performing services exceeding the funding amount authorized in each WAF. Upon final payment and completion of authorized work, any remaining funding is unencumbered, with all invoices for pre-amendment work due by July 15, 2008. The revised WAF (Attachment C-1) specifies work types including spill response, abandoned drum management, overpack drums, sampling, and material transport, with detailed descriptions of personnel, vehicles, equipment, and disposal arrangements required.
