Change Order No. 002 to DEP Contract No. LAB029

This is a Change Order No. 002 to DEP Contract No. LAB029 between the Florida Department of Environmental Protection (DEP), Bureau of Laboratories, and Water & Air Research, Inc., a contractor located in Gainesville, Florida. The contract was originally entered into on April 14, 2008, and has been amended multiple times, most recently on August 5, 2014. This change order modifies the contract terms effective as of the date of execution.

The change order updates the contact information and communication procedures for both parties. Specifically, it deletes and replaces Paragraph 15, which now designates Sara Armour and Kathryn Holland at the DEP Bureau of Laboratories in Tallahassee as the Department's contract managers, along with Dr. David Evans as the contractor's contract manager. It also deletes and replaces Paragraph 16, which specifies the phone numbers for the contract managers: Sara Armour (850-245-8084) and Kathryn Holland (850-245-8095) for the Department, and Dr. David Evans (352-372-1500) for the contractor. All other terms and conditions of the original contract remain unchanged. The change order was executed by both parties in December 2014 and approved by the Chief of the Bureau of Laboratories on January 6, 2015.
