RE: COST PROPOSAL FOR MPE SYSTEM RELOCATION

CONCORD CUSTOM CLEANERS NO. 34

3475 THOMASVILLE ROAD

TALLHASSEE, LEON COUNTY, FLORIDA

DEP FACILITY ID NUMBER: ERIC_4877

PROPOSED TASK ASSIGNMENT: DC020L

Dear Ms. McClure:

Golder Associates Inc. (Golder) is pleased to provide the Florida Department of Environmental Protection (FDEP) with this cost proposal to complete multi-phase extraction (MPE) system relocation activities at the above-referenced site. This proposal includes a description of the proposed scope of services, level of effort sheets for each subtask, and a cost estimate form that summarizes the costs of the proposed services. The proposed scope of services is described below.

SUBTASK 1 – PROJECT MANAGEMENT

To facilitate project quality and satisfactory completion of the proposed scope of work, all aspects of this project will be overseen by an experienced project manager. The project manager will be responsible for the schedule, budget, invoicing, project activities, and communications with FDEP and the field team. Project management will include coordination of field activities; resource allocation to confirm the work is conducted by qualified professionals in accordance with the project schedule; cost control and tracking of the budget status; and submittal of project invoices in accordance with FDEP Drycleaning Solvent Cleanup Program requirements. The project manager will oversee the work so that it is conducted in a manner that is consistent with the requirements of the FDEP-approved task assignment.

SUBTASK 2 – MPE SYSTEM RELOCATION ACTIVITIES

Golder will mobilize to the site to relocate the trailer-mounted MPE system to the FDEP storage yard

(8345 Highway 33, Lakeland, Florida [Fountain RV]). The MPE system (FDEP Property #117310) is no longer in operation and is currently staged next to the soil vapor extraction system, as shown on the attached Figure 6. There is no fencing surrounding the system, and power to the system has already been disconnected. Removal and relocation of the trailer-mounted MPE system will consist of the following general tasks:

Ms. Miranda McClure FDEPDC020 P13-82530

Florida Department of Environmental Protection April 8, 2019

Oversight of the transportation and disposal of a total of approximately 3,600 pounds of spent carbon from the two vapor-phase carbon canisters. Golder collected a composite sample of spent carbon from the two vapor-phase carbon vessels in December 2018 for waste characterization purposes. The sample was analyzed for pH, flashpoint, and VOCs using the Toxicity Characteristic Leaching Procedure. The analytical results from the spent carbon sample indicated the carbon can be disposed as non-hazardous waste.

Loading and transporting the two carbon canisters to Leon Iron and Metal at 1351 Aenon Church Road in

Tallahassee, Florida for scrapping. Golder will obtain a donation receipt from the facility and remove any FDEP property numbers from the canisters prior to scrap.

Transporting and disposing of 30 to 40 gallons of oil located within two plastic containers inside the MPE system. The oil will be disposed at Leon County’s Solid Waste Management facility at 7550 Apalachee

Parkway in Tallahassee, Florida.

Removing the MPE system anchors from each corner of the trailer.

Preparing the MPE system for transportation to FDEP storage yard.

Site restoration in the area of the MPE system.

Preparation of FDEP documentation for system transfer.

Oversight of wrecking service to tow the MPE system to FDEP storage yard.

Dispose of debris and other materials generated during system decommissioning.

The MPE trailer tires currently located inside the trailer will be placed back onto the trailer. If the tires are observed to have deteriorated, new tires will be purchased and a work change directive will be submitted to the FDEP for approval.

Upon completion of the tasks described above, Golder will submit field notes, a photograph log, and a signed copy of the equipment chain-of-custody form to the FDEP via an e-mail deliverable.

HEALTH AND SAFETY

Golder will update the existing site-specific Health and Safety Plan for the field work. The work will be completed in accordance with Golder’s safety policies and procedures. At a minimum, Golder will wear safety glasses, high visibility safety vests, and steel-toed boots. A safety job briefing will be conducted prior to the start of work activities, and anytime the scope of work or site conditions change.

COST AND SCHEDULE

The estimated cost to complete the MPE system relocation is $11,827.71. Golder will tentatively schedule the event for April 2019. Golder anticipates the MPE system relocation activities will take up to three days to complete. The

LOE/CEF cost sheets are included in Attachment A. Subcontractor and vendor quotes are included in

Attachment B. The following table summarizes the proposed costs and deliverable schedule.

Proposal #18937

Carbon Service and Equipment Company is pleased to offer pricing for our spent carbon vacuum extraction service for your site located in Tallahassee, FL, as follows:

(1.) All labor and equipment to remove non-hazardous spent vapor phase carbon from two(2) Air 1,800 adsorbers via vacuum extraction.

(2.) Removal of the spent carbon from site.

(3.) Reactivation of the spent carbon.

(4.) All freight.

(5.) Leon County’s 7.5% sales tax.

The price, complete as described above, will be $3,137.93. Terms are Net 30 Days. Pricing is valid for 30 days.

Pricing is valid if the spent carbon is free-flowing and free of miscellaneous debris and accepted for reactivation.

CSEC has spent carbon acceptance for this site – no additional forms are needed at this time.

The following is required to remove the spent carbon from site for reactivation:

(1.) Completed Reactivation Facility Papers.

(2.) A Toxicity Characteristic Leachate Procedure (TCLP) from a NECLAC lab and no more than 3 months old.

(3.) One (1) - 1 quart sample of the spent carbon sent to our facility.

There is an approximately three (3) – four (4) week lead time on the acceptance of spent carbon for reactivation. This acceptance process begins once CSEC receives the completed forms, spent carbon sample and the TCLP results. Spent carbon acceptance, and subsequent reactivation, is dependent upon the results of the TCLP, the condition of the spent carbon sample and the reactivation facility’s ability to reactivate the site-specific carbon.

I trust that the information provided is adequate for your present decision-making purposes. However, if you have any questions please do not hesitate to contact me.

Thank you for your continued interest in our services.

Very truly, Jennifer Belmore
