About this file

This document is Amendment No. 5 to Agency Term Contract No. GC858, a rate increase amendment executed between the Florida Department of Environmental Protection (FDEP) and FOURTUNE 4, INC D/B/A ENVIRONMENTAL ASSESSMENTS & CONSULTING. The original contract, executed on February 25, 2014, provides petroleum contamination site response action services in the South Region of Florida. The amendment was executed on May 10, 2018, and became effective on May 22, 2018, following the Department's data processing of the rate changes.

The amendment modifies rates for 127 pay items contained in the contract's Rate Schedule (Attachment D, Version 2.2) without adding, deleting, or otherwise modifying any line items. All rate increases are effective only for new work assignments offered on or after the Rate Change Date of May 22, 2018; pre-existing and previously offered work assignments continue to apply the prior rate schedule. The requested rate increases range from approximately 2.27% to 5.00%, with most increases falling between 4% and 5%, and all increases remain within the maximum 5% threshold allowed under contract provision 11.B.ii. The rate changes apply to diverse environmental services including site assessments, drilling and boring, well installation, sampling and laboratory analysis, soil source removal, petroleum storage tank removal, remediation system packages, engineering oversight, and professional services. The contractor submitted detailed justification supporting the increases, which the Department reviewed and approved.
