About this file

Amendment No. 15 to Agency Term Contract No. GC820 is executed between the Florida Department of Environmental Protection (FDEP) and MDM Services, Inc., effective February 20, 2014. The contract provides petroleum contamination site response action services in the North Region of Florida. This amendment modifies rates contained in Attachment D (Rate Schedule V3.5) and establishes a new Rate Schedule (V3.6) to be effective for all new work assignments offered on or after the Rate Change Date of June 10, 2024. All work assignments offered prior to the Rate Change Date will continue to apply the previous rate schedule.

The amendment adjusts rates for numerous pay items across multiple service categories, including mobilization services, drilling and boring operations, sampling and analysis, excavation and remediation, well installation and abandonment, system packages, and professional services. Rate changes vary substantially across line items, with increases ranging up to +5.00% (the maximum allowed under contract provision 11.B.ii) and decreases ranging from -0.41% to -67.98%. Notable significant rate decreases include LDA Rig mobilization items (ranging from -26.97% to -41.58%), several dewatering system items (ranging from -26.97% to -67.98%), and various treatment system packages (ranging from -2.63% to -54.33%). Rate increases of approximately +5.00% were applied to the majority of mobilization, drilling, and professional service line items. The amended rate schedule contains over 400 individual pay items and represents the full scope of services authorized under the contract, with all other terms and conditions remaining unchanged.
