About this file
This document is Amendment No. 15 to Agency Term Contract No. GC807, a rate increase amendment executed between the Florida Department of Environmental Protection (DEP) and Fourtune 4, Inc d/b/a Environmental Assessments & Consulting. The original contract was executed on February 25, 2014, to provide petroleum contamination site response action services in the North Region of Florida. The amendment was executed on May 9, 2024, by Howard Rapaport, President of Fourtune 4, Inc, and Jessica Kramer, Deputy Secretary of the Florida Department of Environmental Protection.
Amendment No. 15 implements rate increases to the contractor's Rate Schedule (Attachment D, Version 3.5) for new work assignments offered on or after the effective Rate Change Date of June 10, 2024. The rate changes apply exclusively to new work assignments and do not affect pre-existing work assignments, which continue under the prior rate schedule. The amendment modifies pricing for approximately 230 pay items spanning multiple categories including mobilization services, drilling and boring operations, well installation and abandonment, soil and water sampling and analysis, remedial action systems, and professional services. Rate increases across all items range from approximately 4.05% to 5.01%, with the majority increasing by 5.00%, which does not exceed the maximum 5% increase authorized under contract provision 11.B. The contractor submitted detailed justification supporting the requested increases, which the Department reviewed and approved. All other contract terms and conditions remain in effect, and the amendment does not add, delete, or modify line items contained in the original contract.
