About this file

This is Amendment No. 5 to Agency Term Contract No. GC750, a rate increase amendment executed between the Florida Department of Environmental Protection (FDEP) and Fourtune 4, Inc d/b/a Environmental Assessments & Consulting. The original contract was executed on February 25, 2014, to provide petroleum contamination site response action services in Florida's Central Region. This amendment modifies the contractor's rate schedule effective for new work assignments offered on or after the Rate Change Date of May 22, 2018, while all pre-Rate Change Date work assignments continue to apply the prior rate schedule.

The amendment increases rates on 104 pay items across the contract's scope of work, with individual rate increases ranging from approximately 3.70% to 5.00%, all within the maximum 5% increase threshold permitted under contract provision 11.B.ii. The revised rates cover comprehensive environmental remediation services including drilling and boring operations, well installation and abandonment, sample collection and laboratory analysis, soil source removal and disposal, remediation system construction and operation, pilot testing, and professional engineering and geology services. Specific examples of rate changes include pay item 1-1 (File Review) increasing from $448.50 to $470.00, pay item 6-12 (Installation of Well Vault) increasing from $1,080.00 to $1,134.00, and pay item 19-11 (Level 1 Remedial Action Plan) increasing from $9,975.00 to $10,400.00. The updated rate schedule is designated as Attachment D (V2.2) and applies only to work assignments offered on or after the effective date, with all other contract terms and conditions remaining unchanged.
