Document Summary
This is Amendment No. 2 to DEP Contract No. GC721, a state and local indefinite delivery contract between the Florida Department of Environmental Protection (FDEP) and LFR Inc. for petroleum contamination site cleanup services. The contract was originally executed on June 21, 2009, and this amendment was executed on July 31, 2009. The contract is funded in part by a Leaking Underground Storage Tank Trust Fund Program grant from the U.S. Environmental Protection Agency (EPA) under Federal Award No. LS-95439109-0. The contractor was selected through a competitive process to perform cleanup services at contaminated sites across Florida.
Amendment No. 2 revises task assignment and task assignment change order forms to incorporate EPA-funded task assignments and related documentation. The amendment adds Attachments A-2 (EPA Funded Task Assignment Form), B-2 (EPA Funded Task Assignment Change Order Form), F-1 (Revised Subcontractor Utilization Report Form), and N (Additional Terms and Conditions for Federal EPA Funds). Services include site remediation activities at the remedial phase (SR) consisting of Free Product Recovery, Corrective Source Removal, Contaminant Source Removal, and Corrective Source Removal activities. The contract establishes fair share goals for minority business enterprises and women's business enterprises at 9% MBE and 3% WBE across construction, supplies, services, and equipment categories. The contract mandates compliance with federal environmental standards, wage and labor requirements, drug-free workplace certification, trafficking prohibition, and various federal procurement regulations outlined in 40 CFR provisions.
