This is a Task Assignment Change Order Form issued by the Florida Department of Environmental Protection (FDEP) for Contract CN213. The document modifies a professional services agreement between FDEP and Genesis, a civil engineering and design firm headquartered in Tampa, Florida. The change order pertains to work at Rainbow Springs State Park and involves FDEP personnel including Philip Stone (Contract Manager), Richard Rowe (Project Manager), and Greg Turral (Budget Representative), along with Genesis representative H. Phillip Keyes, P.E.

The change order extends the completion date for Task Assignment 07 from March 2, 2018 to March 2, 2019—a 365-day extension—due to construction funding constraints in Fiscal Year 2018, with project scheduling revised to align with the FY19 budget cycle. The scope of work and compensation for professional services remain unchanged at a total amount of $31,726.40, which includes a reimbursement amount of $500.00 and a fixed fee of $31,226.40. The project involves hydropneumatic tank, piping, and electrical building replacement at the state park. No additional cost is incurred with this change order, and the original task execution date of August 10, 2017 remains in effect.
