This document is a Task Assignment Notification Form from the Florida Department of Environmental Protection (FDEP) issued to DodStone Architects for the Day Use Restroom ADA Modifications project at Ochlockonee River State Park. The task assignment is part of Contract CN212, with DodStone Architects serving as the consultant and Don Page as the FDEP Contract Manager. The document includes a detailed proposal from DodStone Architects dated July 6, 2021, outlining the remaining tasks related to the construction project that were not completed under the previous task assignment.

The contract covers six specific subtasks totaling $8,999.86, including Limited Construction Administration Services, multiple architectural site visits (interim, substantial completion, and final completion), and an electrical rough-in site visit. The services involve reviewing shop drawings, responding to contractor questions, converting drawing sets, conducting site inspections, and generating field reports. The work is to be completed by June 30, 2022, with all deliverables to be provided electronically. No additional reimbursable costs are included, and the pricing is based on fixed hourly rates for different professional classifications across DodStone Architects and two subcontractor firms: FSM Engineering and Applied Research.
