About this file

This is a CORI Compliance Certification form (Form CM-15A) issued by the City of Boston in May 2025, establishing requirements for vendors contracting with the city regarding Criminal Offender Record Information (CORI) screening policies. The form requires all vendors to certify compliance with City of Boston Code, Chapter 4, Section 7, which mandates fair policies for screening individuals with criminal backgrounds. Vendors must select one of three certification options: confirming they perform no CORI checks on applicants, affirming their CORI policy conforms to standards outlined in Form CM-15B, or disclosing that their policy does not conform to the standards and providing documentation of their written CORI policy. The certification must be completed and signed under penalties of perjury by an authorized representative of the vendor's business.

Vendors who select the third option, indicating non-conformance with Form CM-15B standards, are generally prohibited from entering into contracts with the City of Boston unless a waiver is granted by the awarding authority. Waivers may be granted on a contract-by-contract basis under exigent circumstances, with a completed Form CM-15C required for approval. The form was approved as to form by Corporation Counsel in April 2025 and represents a mandatory compliance requirement for all vendors seeking to conduct business with the City of Boston.
