Category
Sample Problem Identified
Assess
Data

Clinical Infrastructure

· It takes our community members over an hour to get to the nearest primary care facility

· The Tribe has one ambulance to serve 2,000 square miles ()

· GIS Map all the major community locations where the majority of emergency calls come from and their proximity to a health care facility

· Using emergency call data from the previous problem, identify locations where there may be a major route between calls and facility

· Number of Primary Care/Emergency Calls

· Number of Behavioral Health Calls

· Number of miles from places where the most calls derive to the facility

· Number of community hubs where community members gather (e.g.: bingo hall school, stick ball field) and proximity to facility

· Route for emergency vehicle calls most visited (by zip code?)

Workforce Barriers
· Patients wait up to 60 days for an appointment in the behavioral health department

· Patients wait up to 2 hours in the ED for a provider

· Review all the patient surveys for qualitative data regarding wait times

· Track the vacancies for each department

· Survey the staff for feedback on the wait times

· Qualitative analysis of comments from patients

· Vacancy report that calculates number of vacancies by department and time of vacancies

· Qualitative analysis of staff survey

Training
· There is one behavioral health training site in the state and it costs $50,000 a year
· Partner with local TCU to determine if existing course catalog can be revamped for primary care training, behavioral health training, or dental health training

· Survey community members to assess interest in careers in primary, behavioral, or oral health careers

· Identify the number of TCUs within an X mile radius from Tribe’s community

· Identify the number of programs/majors offered that focus on primary care, behavioral health, and/or oral health

· Analyze the number of community members expressing interest in careers

· Number of partners in education and training within the community

· Total cost of training programs identified

· Projected cost to update the curricula identified

Cultural Inclusion
· Patients prefer to include traditional medicine in their treatment plan but we can’t bill for those services
· Identify all the methods of traditional medicine appropriate by Tribal custom for use in primary care, behavioral health, and oral health

· Partner with the state Medicaid to identify how these can be incorporated into a treatment plan

· Partner with local clinicians to seek pathways to include in potential treatment plans for common health issues.

· Number of traditional practices appropriate (as determined by the Tribe) for primary care, behavioral health, and dental health

· Number of common health issues seen in the community (e.g.: cavities, substance misuse, inflammation of muscles)

· Number of clinicians on workgroup to identify how the practice can be incorporated to treatment basic health issues
