Re: New Well; Replace Surface Water Source (PR#2020-68) USFS Shadow Bay Campground (PWS ID#OR4192815) Conditional Approval

Dear Mr. Caswell:

Thank you for submittal to the Oregon Health Authority’s Drinking Water Services (DWS) of plan review information for installation of a water supply well to replacement the existing surface water source at USFS Shadow Bay Campground. On 5 May 2020, DWS received a fee of $825 to complete the plan review and plans were submitted 20 May 2020. DWS grants the project conditional approval.

The project involves installing a well 100 to 300 feet below grade in the vicinity of the existing pump building. Once functional, the well supply will provide water to existing infrastructure and the surface water source will be abandoned. The plans are approved conditional on the following:

1. There is a sample tap in the pump building per OAR 333-061-0050(2)(k)(v);

2. There are appurtenances that allow the entire flow from the well to be pumped to waste per OAR 333-061-0050(2)(k)(vi);

3. There is a means to measure flow rate per OAR 333-061-0050(2)(k)(vii); and

4. Analytical results for total coliform bacteria, nitrate, and arsenic.

An OHA geologist wrote: “The well is to be located in what is surficially mapped as Quaternary basaltic lava flows. Placement of the well seal shall be in accordance with but not limited to OAR 690-210-0150 (sealing of wells in consolidated formations) the depth of casing seal placement shall depend on the lithology encountered during drilling and depth of the 1st

USFS Shadow Bay Campground (PR 2020-68) 11 August 2020 Conditional Approval water bearing zone. The seal shall extend 5 feet into the consolidated layer directly overlying the water bearing zone. The aquifer nature cannot be directly characterized due to a lack of completed wells in the area. However, based on the volcanic nature of the mapped geologic units in the area; the aquifer is likely to be confined and overlain with consolidated material.

The new well location shall meet all sanitary hazards setbacks as per OAR 333-061-0050 (2) (a)(E). The as built well log shall be submitted for OHA review once completed.”

Until DWS receives verification that the conditions have been met and final approval has been issued, the well is not approved for use. Documentation demonstrating how the above conditions are met should reference Plan Review #2020-68 and can be emailed to me at james.r.macpherson@dhsoha.state.or.us or mailed to:
