About this file
The document is an Attachment 9 - Proposed Subcontractors form for Solicitation No. BPM006694, issued by the Arizona Department of Administration State Procurement Office. The solicitation is specifically for Weapons, Lethal, Less than Lethal, and Supplies. The form provides a template for contractors to disclose potential subcontractors, with space for up to five subcontractor entries, including their contact information, small business status, work to be performed, and quality assurance/quality control measures.
The solicitation allows contractors to either indicate they will perform all work with their own personnel or list proposed subcontractors. For each potential subcontractor, the form requires detailed information including the type of work they will perform, their approximate percentage of total contract work, and copies of relevant certifications. The document suggests that the state may request additional information about proposed subcontractors as a precondition of contract award. While specific pricing, option periods, or contract amounts are not detailed in this attachment, the form is designed to provide transparency about the potential workforce composition for the weapons and supplies procurement.
