About this file

This document is a Worker Protection and Investment Certification Form issued by the Commonwealth of Pennsylvania under Executive Order 2021-06. The certification requires contractors and grantees to verify compliance with numerous state labor and workforce safety laws, including acts related to workplace misclassification, employment of minors, minimum wage, prevailing wage, equal pay, and various other labor regulations. Contractors must certify their adherence to these laws and understand that false certifications can result in contract termination and potential three-year debarment from future contract bidding.

The certification form establishes legal accountability for contractors, with penalties for providing false information under 18 Pa. C.S. § 4904. Contractors must sign and date the document, providing their name, title, and organization, and commit to immediately reporting any changes in compliance status to the purchasing agency. The document specifically outlines 13 distinct labor laws and workplace safety regulations that contractors must follow, demonstrating the Commonwealth's comprehensive approach to worker protection and workplace standards.
