C.S. § 531 (Debarment or suspension). The certification is subject to the penalties outlined in 18 Pa. C.S. § 4904 (Unsworn falsification to authorities). The related contract opportunity pertains to the provision of produce products for the Office of Mental Health and Substance Abuse Services (OMHSAS) covering the period of October through December 2026. The certifying official executing this form must be duly authorized to do so on behalf of the contractor or grantee entity.
