STATEMENT OF WORK
Inspection, Safety Device Testing and Calibration
Boiler Plant Burners, Controls, Instruments, and Data Management
1. Introduction:
A. The Amarillo Veterans Affairs Medical Center (AVAMC) has a central boiler plant to provide steam for heating and air conditioning, domestic hot water, kitchens, and sterilization.
B. This contract is generated to comply with VA Directive 2008-062.
C. Boiler plant safety, reliability, efficiency, and equipment longevity are paramount concerns of VAMC management. Malfunctioning burners or controls can cause catastrophic events resulting in injuries and death and massive property damage. Inaccurate or inadequate data management and monitoring systems can fail to warn of unsafe or inefficient performance. Regular inspections, testing and calibration by expert technicians are an essential part of a program to address these concerns.
D. This “Statement of Work” describes the requirements for the inspection, testing and calibration of the boiler plant burners, controls, and instruments by qualified technicians.
E. A separate document lists the facilities where the work will be required, lists the boilers at each facility, and provides a schedule for the work.
F. A separate “Statement of Work” document covers the inspections of the boilers and boiler plant equipment by Qualified Professional Inspectors.
2. Definitions/Acronyms
A. CO - Contracting Officer: The Contracting Officer is a person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings. The term includes certain authorized representatives of the Contracting Officer acting within the limits of their authority as delegated by the Contracting Officer.
B. COTR - Contracting Officers Technical Representative: Is responsible to furnish technical guidance and advice or generally monitor the work performed under the contract.
Representatives will not be authorized to make any commitments or changes, which will affect the price, quantity, and quality or delivery terms. A Contracting Officer acting within his/her authority is the only authorized Government employee to execute all changes to a contract.
C. PMI – Preventive Maintenance Inspection: Services which are periodic in nature and are required to maintain the equipment in such condition that it may be operated in accordance with its intended design and functional capacity with minimal incidence of malfunction or inoperative conditions.
3. Technician Qualifications:
A. Technicians shall have completed at least a one-year trade school and have five years successful experience in this field. The experience shall be largely with institutional and industrial boiler plants similar in design to the VAMC plant. The VAMC facility manager/engineer may define and accept equivalent qualifications.
B. Technicians shall demonstrate familiarity with and ready access to the current versions of the following references:
NFPA 85, Boiler and Combustion Systems Hazards Code.
VHA Boiler Plant Safety Device Testing Manual, 3rd Edition.
C. Technicians shall be equipped with portable electronic flue gas analyzers and other test instruments necessary for the required tests and calibrations, all calibrated within one month of the site visits. At facilities with programmable digital controls, the technicians must be capable of programming the controls and have the appropriate hardware and software for this.
4. Scope Of Work
A. The Contractor is subject to the application of VA Directive 0710 titled Personnel Security and Suitability Program from June 4, 2010
B. Inspection, Testing and Calibration Requirements:
a. Technicians shall provide all tools and labor necessary to perform inspection, tuning, testing, calibrating, and adjustments of burners and boiler and boiler plant controls as specified below. This shall be done every six months in accordance with a schedule provided by VAMC. VAMC may extend the schedule for burners in limited service.
b. Instrumentation, monitoring and data management systems as listed below shall be calibrated every six months.
c. Provide at least two weeks’ notice to the Contracting Officer’s Technical Representative (COTR) prior to performing the work. Work cannot be scheduled during heavy steam load periods. Only one boiler at a time can be out of service for the inspection, testing and calibration procedures. Work must be conducted when the COTR or his/her designate is available on site to monitor the work.
d. The Amarillo VAMC will have the boilers that are to be serviced prepared for the technicians upon their scheduled arrival. This includes having the boilers clean of soot and loose scale;
fully warmed and at normal steam pressure; steam exhaust silencer system operable; all boiler, burner and fuel train pressure gages and thermometers calibrated; fuel meters in accurate operation (pressure/temperature correction factors provided, if applicable); boiler steam flow, stack temperature and flue gas oxygen instruments operating.
e. The Amarillo VAMC will comply with any other requirements of the test personnel that are considered reasonable by the COR and have been presented in writing at least two weeks prior to the scheduled testing.
C. The inspections, testing and calibrations shall comply with:
a. The recommendations and requirements of VA Directive 2008-062 Boiler Plant Operations
b. The written recommendations of the equipment manufacturers.
c. The requirements and recommendations of NFPA 85 Boiler and Combustion Systems
Hazards Code including applicable appendices.
d. Burner performance requirements in this document.
D. A summary of the work is as follows:
a. Review boiler plant log sheets and alarm and trouble reports.
b. Review records that show combustion performance (flue gas oxygen and carbon monoxide).
c. Perform overall visual inspection of systems. Verify that systems comply with referenced codes and VAMC requirements stated in this contract.
d. Test and record the operation and set points of all burner/boiler safety interlock devices.
Refer to list below. Verify that the set points and operating points are within approximately 20% of normal operating parameters. Make adjustments as necessary and record the new settings. The operation of a device must result in burner shutdown and/or proper alarm operation.
e. Operate burner(s) on each fuel from low fire to high fire and back to low fire in at least six increments and record combustion performance (flue gas oxygen, carbon monoxide, NOx where applicable), fuel train pressures, atomizing train pressures, burner pressures, stack temperatures, boiler steam output.
f. Compare the combustion performance data with VAMC requirements (see below) and previous readings. If necessary, make adjustments to the fuel flow and combustion air controllers, control valves and dampers to obtain the required performance. Record the new performance data.
g. Verify accuracy of instrumentation listed below. Verify that all devices are properly selected for the application in terms of type, size, set point range, performance, code approval.
Calibrate all instruments that are not within manufacturer’s specifications for accuracy.
h. Immediately inform COTR of any recommended repairs or modifications.
E. All of interlocks and safety devices to be inspected and tested on each boiler/burner/equipment, are contained in the VHA Boiler Plant Safety Devices Testing Manual, 3rd Edition, along with testing procedures.
F. Required burner performance (natural gas and fuel oil):
a. Turndown (ratio of maximum and minimum firing rates): 10/1 8/1 5/1 4/1 (Refer to original burner specification).
b. Achieve, but do not exceed, boiler maximum steam flow output rating. Measure fuel input at minimum and maximum firing rates.
c. Maximum carbon monoxide: 200 parts per million (ppm)
d. Maximum NOx: _____ (Refer to original burner specification).
e. Flue gas oxygen: 2.5 – 4.2% (Up to 5.2% at loads below 40% of maximum steam output; no upper limit at minimum firing rate; oxygen can be one percentage point higher on oil firing on single-point positioning systems).
f. Flue gas oxygen (low excess air burners): 1.0 – 2.0% (Up to 2.5% at loads below 40% of maximum steam output; no upper limit at minimum firing rate; oxygen can be one percentage point higher on oil firing on single point positioning systems).
g. No visible smoke, except on heavy oil fuel maximum opacity is 20%. Comply with local emissions regulations.
h. Editors Note: Copies of the applicable emissions regulations must be available in the boiler plant office.
i. Flames shall be stable with no pulsations, shall be retained near burner, no blowoff or flashbacks, no constant flame impingement on refractory or waterwalls.
G. List of instrumentation and controls to be inspected and calibrated:
a. Steam flow transmitters (all).
b. Steam flow recorders/computer readout.
c. Flue gas oxygen sampling, analyzing and recorder/computer readout.
d. Boiler and economizer stack temperature transmitters and recorder/computer readout.
e. Master steam pressure and combustion controllers.
f. Boiler outlet draft controllers.
g. Boiler water level controllers.
h. Feedwater deaerator and condensate storage tank water level controllers including overflow.
i. All pressure and temperature sensors and transmitters.
j. All signal processing and readout devices.
5. Report Requirements:
A. Provide complete written report of the inspection fully describing all tests performed, all findings, and recommendations. The report shall have two sections the first section will be in a table form that with four columns:
a. The first column labeled “FUNCTION” did the device pass or fail.
b. The second column labeled “Device” list the device number and description in accordance with the VHA Boiler Plant Safety Devices Testing Manual, 3rd Edition.
c. The third labeled “Deficiencies-Recommendations”
d. The fourth column labeled “corrective actions” list any corrective actions taken during the inspection.
The next section shall be in paragraph/bulleted form and list each device individual in accordance to the VHA Boiler Plant Safety Devices Testing Manual, 3rd Edition. Plus the section must reference the correct Master Specification and Standard Detail for that device. See sample below.
5.1. Condensate Storage Tank High Water Alarm
Purpose: High water level may be an indication of condensate transfer pump failure that could lead to low water condition in the feedwater deaerator and in the boilers.
There may be a failure of make-up water controls.
Recommended set point: 4 inches below overflow level and 2/3 of tank height.
Potential hazards due to failure of high water alarm: Feedwater deaerator running dry and, consequently, boiler feed pumps running dry will cause overheating and damage to pumps and potential for plant shut down. Failure of make-up water controls (in open position) could lead to significant cost from waste of water and condensate from the tank overflow if this occurs over a long time period.
Recommended type of device: Conductivity probe type switch. Float type switches have high failure rate in this application where they are exposed to flash steam that causes seals to harden and prevent float movement.

Comments: Float type switches are not allowed for high water alarms. The VA recommended switch type is a conductivity probe type switch. Install the VA recommended switch and set to alarm at 2/3 of the tank height. There are valves that can isolate this alarm. These valves must be made lockable only in the operating position.
B. Furnish report within five (5) working days of each facility inspection in “Microsoft Word” format by email to the COTR and to the VA Headquarters office designated. Provide hard copies of data sheets and flue gas analyzer “strip” printouts to the COR within five (5) working days of visit.
C. All safety-related deficiencies shall be immediately reported to the COR, Boiler Plant Supervisor and/or Chief Engineer during the inspection visit.
D. Contractor shall report and brief the COTR on the conditions and performance of the boilers after finishing the work and leaving the facility.

7. Addition or Removal of Boilers
A. The Government reserves the right to add or remove boilers from this contract at any time. The COTR will inform the Contractor as far in advance as possible for any changes in the inventory in writing and via the Contracting Officer.
B. If the Government removes a boiler from service to perform work outside the scope of the contract, the Government will deduct the amount currently being paid to test and tune that boiler.
When the Government returns the boiler to service, payment for that boiler will resume.

9. Safety
A. The Contractor shall adhere to all OSHA, EPA, NFPA Life Safety Codes, and all other regulatory requirements.
B. In performance of this contract, the Contractor shall follow VAMC safety policy and standard for safe work practices and take such safety precautions as the Safety Officer or designee may determine to be reasonably necessary to protect the lives and health of occupants of the building. The Contractor shall comply with VAMC smoking policy, which designates all interior space as non-smoking areas.
C. Patient, employee, visitors, and contractor personnel safety shall be maintained at all times. The contractor is responsible for the occupation safety and health of his/her employees. The Contractor shall comply with all applicable OSHA safety and health standards. The Contractor shall replace all safety guards, equipment, devices, etc. removed to service or repair the elevator immediately after completion of work or when leaving the job unattended.
D. The Contractor shall submit to the COTR, prior to the start of the contract, the Material Safety Data Sheets (MSDS) for all potentially hazardous materials (lubricants, cleaners, working fluids, etc.) to be used in the facility in performance of the contract, and will not use, in the facility, such materials which have not been so cleared in advance with the VAMC. MSDS for new chemicals shall be furnished concurrently with arrival of the chemical on site. The Contractor shall maintain a copy of all MSDS at the chemical storage site in a location accessible to VAMC personnel to assure compliance with all laws and requirements regarding the “Right to Know” law.
E. The Contractor is responsible to identify, provide and maintain all personal protective equipment required to perform the duties outlined in the contract. In addition, the contractor is responsible for identifying and providing all applicable safety programs (i.e. lockout/tag out, confined space entry, universal precautions, etc.) required to perform the work. Training on required safety programs and the proper use of PPE’s shall be provided, and documentation maintained by the contractor.
F. The Contractor shall obtain a Hot Work Permit from the Safety Office whenever soldering, welding, using a cutting torch, or other open flame, spark, or heat producing equipment. The contractor is required to follow ALL requirements outlined for the issuance of the Hot Work Permit.
G. The Contractor shall be notified of any non-compliance with the pervious provisions. The contractor shall, after receipt of such notice, immediately correct the condition and notify the contracting officer in writing that the condition has been corrected. It, in the opinion of the Safety Officer or designee the condition is life threatening he/she shall instruct the contractor to stop work until the condition has been corrected. If the Contractor fails or refuses to comply promptly, the Contracting Officer may issue and order halting of all or any part of the work, and Contractor may be held in default. The Contractor bears all costs of stop work time and correcting safety hazards.
H. The contractor shall provide within ten (10) working days after receipt of this contract a detailed listing of the following information:
a. Safety program/procedures are required to be followed by the contractor’s personnel in the performance of their duties and when such procedures are required.
b. Listing of personal protective equipment(s) required to be utilized by the contractor’s personnel in the performance of their duties. Also when equipment will be required.
c. Material Safety Data Sheet (MSDS’S) for any chemical(s) utilized by the contractor in the performance of this contract.
d. Contractor is responsible for the supervision of all their employees while on government property. It is the Contractor’s responsibility to assure compliance with the scope of work and requirements referenced in this contract. Provisions provided in the scope of work are not intended to relieve the Contractor of this responsibility.
9. Quality Control Program
A. The Contractor shall establish a complete quality control program to assure the requirements of the contract are provided as specified, Within five (5) workdays prior to the starting date of the contract, the Contractor shall submit a copy of his program to the COR, for approval. The program shall include, but not be limited to, the following:
a. An inspection system covering all the services described in the contract. A checklist used in inspecting contract performance during regularly scheduled or unscheduled inspections.
The name(s) of the individual(s) who will perform the testing.
b. The checklist shall include every area of the Contractor’s operation as well as every task required to be performed.
c. A system for identifying and correcting deficiencies in the quality of services before the level of performance becomes unacceptable and/or the Medical Center inspectors point out the deficiencies.
d. A file of all inspections conducted by the Contractor and the corrective action taken. This documentation shall be made available to the Medical Center upon request, at any time during the term of the contract.
10. Emergency Telephone Numbers
A. The Contractor shall provide the COR with pertinent emergency telephone numbers, in order to summon assistance in case an emergency develops. At least one of the emergency telephone numbers shall be manned twenty-four (24) hours per day. This information shall be provided in writing to the COR, prior to the contract start date.
11. Information Relating to Conduct of Contractor’s Employees

The preliminary notice does not commit the Government to an extension.
(b) If the Government exercises this option, the extended contract shall be considered to include this option clause.

SUBCONTRACTING—CERTIFICATE OF COMPLIANCE FOR SERVICES AND
CONSTRUCTION (SEP 2021) (DEVIATION)
(a) Pursuant to 38 U.S.C. 8127(k)(2), the offeror certifies that—
(1) If awarded a contract (see FAR 2.101 definition), it will comply with the limitations on subcontracting requirement as provided in the solicitation and the resultant contract, as follows:
(i) [] Services. In the case of a contract for services (except construction), the contractor will not pay more than 50% of the amount paid by the government to it to firms that are not VIP-listed SDVOSBs as set forth in 852.219-10 or VOSBs as set forth in 852.219-11. Any work that a similarly situated VIP-listed subcontractor further subcontracts will count towards the 50% subcontract amount that cannot be exceeded. Other direct costs may be excluded to the extent they are not the principal purpose of the acquisition and small business concerns do not provide the service as set forth in 13 CFR 125.6.

The offeror further acknowledges that this certification is subject to Title 18, United States Code, Section 1001, and, as such, a false, fictitious, or fraudulent certification may render the offeror subject to criminal, civil, or administrative penalties, including prosecution.
(3) If VA determines that an SDVOSB/VOSB awarded a contract pursuant to 38 U.S.C. 8127 did not act in good faith, such SDVOSB/VOSB shall be subject to any or all of the following:
(i) Referral to the VA Suspension and Debarment Committee;
(ii) A fine under section 16(g)(1) of the Small Business Act (15 U.S.C. 645(g)(1)); and
(iii) Prosecution for violating section 1001 of title 18.
(b) The offeror represents and understands that by submission of its offer and award of a contract it may be required to provide copies of documents or records to VA that VA may review to determine whether the offeror complied with the limitations on subcontracting requirement specified in the contract. The Contracting Officer may, at their discretion, require the Contractor to demonstrate its compliance with the limitations on subcontracting at any time during performance and upon completion of a contract if the information regarding such compliance is not already available to the Contracting Officer. Evidence of compliance includes, but is not limited to, invoices, copies of subcontracts, or a list of the value of tasks performed.
(c) The offeror further agrees to cooperate fully and make available any documents or records as may be required to enable VA to determine compliance with the limitations on subcontracting requirement. The offeror understands that failure to provide documents as requested by VA may result in remedial action as the Government deems appropriate.
(d) Offeror completed certification/fill-in required. The formal certification must be completed, signed, and returned with the offeror’s bid, quotation, or proposal. The Government will not consider offers for award from offerors that do not provide the certification, and all such responses will be deemed ineligible for evaluation and award.

(d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records—Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the
Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor
Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the
Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.
(e)(1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c), and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial products or commercial services.
