About this file
The document is a set of required affidavits for bidders, offerors, and contractors in Kentucky, issued by the Commonwealth of Kentucky. These affidavits include three separate forms: a general affidavit for campaign finance law compliance, a resident bidder status affidavit, and a qualified bidder status affidavit. Each form requires the bidding entity to swear under penalty of perjury to various legal and operational requirements, such as being authorized to transact business in the Commonwealth, having filed Kentucky corporate taxes, made unemployment insurance payments, maintained workers' compensation policies, and meeting specific criteria for resident or qualified bidder status.
The affidavits are designed to ensure that bidding entities meet specific legal and operational standards before being considered for state contracts. The forms allow bidders to claim different statuses, such as a resident bidder (which requires one year of Kentucky business operations and tax payments) or a qualified nonprofit bidder (particularly for organizations supporting individuals with severe disabilities). The Commonwealth reserves the right to request documentation supporting these claims and may disqualify bidders or terminate contracts if the claims cannot be substantiated. Each affidavit must be signed by an authorized representative, dated, and notarized, and remains valid for one year from the date of execution.
